North Tees and Hartlepool NHS Foundation Trust provides hospital-based services to the population of Hartlepool, Stockton on Tees and parts of East Durham and Sedgefield as well as community, therapeutic and screening to a wider population across Durham and Teesside.
Job overview
The post holder will assist in the provision of an efficient, effective and comprehensive administrative service supporting the ward based team. The post holder will be expected to work alongside Patient Flow / Integrated Discharge Team as well as external partners to facilitate flow of patients through the wards and facilitate discharges. The post holder will adopt the use of current discharge processes to ensure safe, timely discharges.
They will report to the clinical care coordinator for the integrated discharge team. Also working closely with the Senior Clinical Professional to implement any changes within discharge processes
Using appropriate technologies and equipment the post holder will produce all work outputs to a high standard meeting the organisations requirements for Patient Safety, Information Governance and CQC recommendations in adherence with Trust policies, procedure and guidelines.
Main duties of the job
The post holder will be expected to communicate and work effectively with all members of the work team, including managers and medical staff, liaising with other departments and internal service users across the trust and external service users e.g. patients, family, care providers, ISPA, etc, ensuring confidentiality at all times

Detailed job description and main responsibilities
- Update of Patient Discharge Tracking List (OPtica)
- Access patient details using the trust electronic systems and Trakcare
- Ensure patient flow is maintained and support electronic system update re patient’s location
- Monitoring of transport, bookings and ensuring patients are discharged in timely manner, and using appropriate transport methods
- Liaise with patient flow team regarding expected patient discharges and promoting the use of the Transport Hub.
- Escalate issues to Clinical Care Coordinators for them to review and aid as required to help facilitate patient flow.
· Demonstrate well developed written/verbal/listening communication skills
· Demonstrate good organisational / time management skills and be able work under pressure to meet deadlines.
· Demonstrate ability to work on own initiative and also to work as part of a team.
Demonstrate willingness to be flexible and to work in other clinical areas throughout the trust as and when required in accordance with operational service needs.
