Community Staff Nurse / Registered Practitioner - HCP
Band 5
Job overview
We have an exciting opportunity for enthusiastic and conscientious practitioners to join our 0-5 team based in Bognor Regis (working across Arun district i.e. Bognor Regis and Littlehampton). Our service is open Monday to Friday 9-5. Previous experience of working in the community is not essential.
Are you a qualified or about to qualify (child, adult, mental health or learning disability) nurse or a midwife with a passion for public health and improving outcomes for children in West Sussex? Would you like to work for a friendly and passionate Health visitor team who make a difference to children’s lives every day?
Alongside a skill mix team of health visitors and nursery nurses, you will work in collaboration with families; providing universal and targeted early interventions to ensure every child is offered the best start in life.
We are looking for a compassionate individual who can demonstrate excellent communication skills and is able to work in partnership with multi-agency and inter-professional colleagues to support the delivery of the Healthy Child Programme (HCP).
This is a permanent position but may also be an excellent opportunity to gain experience for an individual interested in undertaking a Specialist Community Public Health course in health visiting or school nursing in the future. A programme of support is being developed to support internal applicants interested in applying for a funded degree to become a health visitor or school nurse.
Main duties of the job

- Undertaking holistic assessments, identifying health needs, parenting capacity and family/environmental factors
- Implementing and evaluating programmes of care

- Undertaking targeted support in line with our commissioned service
- Inter-professional working to deliver the healthy child programme and safeguard children
