Home First Occupational Therapist

The Home First service brings together nurses, therapists, health and therapy assistants, to work with the acute Multi-Disciplinary Team & Transfer of Care Hubs to promote an efficient, effective discharge from hospital to the patient’s own home. 

Our multidisciplinary teams work flexibly to provide a 7-day service, 365 days a year, between 8am and 8pm. Patients who are discharged from hospital into Home First will be receiving step-down intermediate care, involving community-based assessments and interventions provided in their own home to support recovery and monitoring of their health in the early days post discharge to prevent re-admission. Home First will, where required, provide acute reablement for up to 10 days, supporting people to achieve functional goals and work towards independence with activities of daily living.  

You could become part of our exciting service, bringing the unique benefits of a wraparound recovery service from hospital discharge. The principle of the service ensuring that recovery involves working towards the person being as independent as possible through enablement.

Prif ddyletswyddau'r swydd

 

a)     To work effectively as a team member in providing and facilitating a high standard of therapeutic, social, and rehabilitative support in the unit, department or in the community. 

b)     To work within the conditions of a rotation if required. 

c)     To provide assessments that appropriately inform therapy, social and rehabilitative pathways taking account of the fluctuating needs of the patient and assist in evaluating the outcomes. 

d)     To undertake a holistic assessment of all the patients’ needs and understand the routes for escalating issues that are not directly resolved by own profession. 

e)     Formulate and deliver an individual treatment programme based on a sound knowledge of evidence-based practice and treatment options using specific clinical assessment, reasoning skills and knowledge of treatment skills. 

f)      To report and document variances in a patient’s condition or planned pathway of care. 

g)     To offer advice, support, and education on healthcare to both patients and carers. 

h)     To take referrals for services using the criteria specified in the team’s operational policies. 

i)       Continually evaluate patient progress, reassess, and alter treatment programmes as required. 

j)       Assess patient understanding of treatment proposals, gain valid informed consent and have the capacity to work within a legal framework with patients who lack capacity to consent to treatment.

Qualifications

  • Professional qualification such as a degree or equivalent in Occupational Therapy
  • Current registration with the Health and Care Professions Council as a Occupational Therapist
  • Evidence of ongoing education

Experience

  • Relevant postgraduate or undergraduate experience in the clinical field
  • Experience within specialism underpinned by theory and specific therapeutic knowledge
  • Experience of using assessment tools relevant to client group

Skills and Knowledge

  • Able to assess, plan, implement and evaluate care for individuals
  • Good communication and negotiation skills
  • Able to demonstrate competency in a range of clinical skills
  • Knowledge of clinical governance, audit, risk management and professional code of conduct
  • Evidence of CPD and commitment to ongoing learning
