We have an exciting new post for the Falls Prevention Brighton and Hove team, part time 24 hours per week. This role involves a variety of duties as per below:

a)    To identify and carry out the specialist assessment of individuals who may be at risk of falls and fracture.
b)    To plan and provide programmes of care according to a person’s individual and complex needs and without direct supervision in a variety of settings, including clinics, exercise groups, health promotion, and education
c)    To manage a caseload of patients utilising specialist clinical assessment, diagnostic and treatment skills in the management of patients at  risk of falls
d)    To provide training and education on falls prevention to a broad range of staff working across the local health and social care sector, including Residential and Home Care Services, and voluntary organisations
e)    To supervise assistant staff/support workers and undergraduate students and provide training to staff and carers
f)    To develop strong links with the Primary Care Trust, Sussex Ambulance and the Voluntary and Independent Sector and local community to deliver an effective service
g)    Adhere to professional standards and guidelines of practice, education and conduct

Prif ddyletswyddau'r swydd

Main tasks:

a)    To support the Falls and Fracture Prevention Service Manager in the continuous monitoring and evaluation of the Falls Service across Brighton and Hove, according to targets set by the commissioning organisation 
b)    To support the manager in the evaluation of the service by carrying out clinical audit included within the team’s clinical effectiveness/audit plan for each year
c)    To maintain up to date data entry according to Trust policy, including activity/contact data using computer-based systems including PIMS/DISCO or equivalent system 
d)    Develop links with the Fracture Prevention Specialists, Consultant Geriatrician and Royal Osteoporosis Society to ensure a coordinated approach to delivering information and advice relating to Fracture Prevention.
e)    To liaise with consultant Geriatricians  regarding the diagnostic falls clinic and plan and prioritise appropriate referrals to the diagnostic falls clinic and individual consultants
f)    To carry out other tasks and roles commensurate with role and as requested by Osteoporosis and Falls Prevention Service Lead 
g)    To support the Falls and Fracture Prevention Service Leads in the development of the service, in particular the development of care pathways, policies and procedures relating to falls prevention following principles of the Calderdale Framework
