Job overview

A very exciting position has opened within our IBD service on the Worthing Site.

The IBD Clinical Nurse Specialist (CNS) will provide a highly specialist nursing contribution to the clinical management and education of patients referred to the service in collaboration with the multidisciplinary gastroenterology team across hospital settings.

You will be a clinical expert within the field of IBD, and a role model to others demonstrating high standards of quality care, facilitation and clinical leadership provision to others.

The role will require intensive training with both internal and external courses. The banding for this position is very much dependant on the level of training and is a progression role starting at a band 6 and progressing to a band 7 once a level of training is completed. Support will be provided to obtain this training and ongoing leaning.

We welcome applications from those who currently hold a UK NMC registration. If you currently hold an overseas nursing qualification and require sponsorship we are not currently direct hiring Internationally Qualified Nurses.

Main duties of the job

Acts as a clinical expert in the development of clinical nursing services with the field of IBD, implementing National strategies at a local level and contributing towards strategic decision making, and implementing policies appropriate to the speciality.

Provides expert clinical advice and education to patients promoting a culture that supports self-management and health and wellbeing.

Detailed job description and main responsibilities

  • To provide an autonomous specialist nursing service to IBD patients by reviewing referrals and delivering holistic care to patients.
  • Work in partnership with the patient and their family/carers to promote self-care and independence wherever possible through education and the use of self-management plans.
  • Act as a resource to colleagues across primary and secondary care settings organising and participating in training programmes as appropriate.
  • Provides IBD nursing clinical expertise, specialist advice and support across service boundaries.
  • To actively integrate theory and practice.
  • Make referrals to other professionals and sign-posts to other agencies as appropriate.
  • Makes clinical judgements based upon experience and knowledge and takes appropriate action.
  • Request diagnostic tests within agreed sphere of competence and make clinical judgements based upon clinical expertise.
  • Assess and manage symptoms, problems related to area of expertise and recognising when appropriate for onward referral is required.
  • To facilitate the assimilation of complex clinical information regarding diagnosis, staging investigations and treatment options in collaboration with specialist consultants.
  • Support and empower patients and carers to influence and participate in often difficult decisions regarding the panning and delivery of care.
  • Promotes self-management, undertaking risk stratification to determine those who can self-manage, following education, those who will need guided support to self-manage and those who will require on-going, face to face support.
  • Assess the special needs of patients referred to the service and facilitate the planning and provision of care, making use of all resources.
  • Undertake clinical supervision of individuals on an individual or group basis.
  • Disseminate information on National guidelines for management of relevant conditions to appropriate multidisciplinary team members.
  • Work within an agreed set of patients’ management protocols and guidelines including national guidance.
  • Develop and participate in Nurse led clinics according to the needs of the service and within the Scope of professional practice.
  • Participate in and initiate research and audit projects.
  • Participate in service development and implements evidence based practice.
  • Check clinics daily for capacity and ensure all slots are filled in a timely manner and supporting the setting up of additional clinics as able
  • Management of specific projects and for supporting divisional projects that contribute to service improvements aimed to ensure that the patient is central to the delivery of high quality patient care

Qualifcations / Experience

  • Registered Nurse
  • Substantial post registration experience with significant experience at a senior level.
