Job overview

This is an exciting and unique opportunity to be at the forefront of the delivery of our hospital intervention team at Eastbourne District General Hospital.

The hospital intervention team (HIT) is a multi disciplinary team based in the gateway wards of the acute hospital. The team consists of nurses, occupational therapists and physiotherapists with cross competencies to assess patients for discharge. The gateway wards include AandE, MAU, CDU as well as others.

The purpose of the senior assistant role is to review and assess patients to ensure they can return home. The post involves conducting social and mobility assessments and completing referrals onto different community services as appropriate for discharge. The successful candidate will be given support and training to have the require competencies to complete these assessments, referrals and issue equipment appropriate to the role. 

Main duties of the job

To conduct home environment assessments and prescribe equipment as deemed appropriate. 

To independently assess and conduct less complex patient reviews (eg stairs assessments), prescribe equipment, review patients who are awaiting intermediate care rehabilitation in order to maximise their rehabilitation potential and to support with discharges to their home environment.

To independently complete referrals to the community as required to support with ongoing community rehabilitation.

Qualifications

Essential criteria
  • Foundation degree in health and social care or equivalent level 5 qualification plus A levels and GCSE or equivalents including functional skills level 2 Or As a level 5 apprenticeship such as AP apprenticeship Or Care certificate

Experience

  • Able to demonstrate substantial experience in health and care settings including in the relevant field of work.
