Job overview
The Discharge Lounge is an integral part of the hospital, supporting patient flow and facilitating safe and timely discharges to patients’ own homes or the next stage in their care journey. The role will include supporting the Registered Nurse (RN) with safe discharges and liaising with members of the multidisciplinary team (MDT) to ensure coordinated, patient-centred care.
Main duties of the job
· Promote dignity, respect and privacy in the delivery of care duties
· To undertake patients’ vital signs, blood pressure, pulse, pulse oximetry, height, weight, neurological assessment, ECG, respirations, urinalysis, blood glucose,escalate concerns in line with policy
· Recognition of deteriorating patient and escalation process
Provide delegated clinical procedures such as removal of cannula, wound assessment and simple dressings
You must be able to work as part of a team, use your own initiative with effective interpersonal and communication skills, contributing fully as a member of the multi-disciplinary team. We will provide a supportive atmosphere, mentorship and clinical supervision on the unit and are committed to the professional development of all staff. Team working is an integral part of this unit with all staff enabled and empowered to make a real difference for their patient.

Qualifications / Training
Essential criteria
- GCSE English and Maths grade C or 4 or Functional skills for English/Maths
- Care Certificate
Desirable criteria
- Evidence of mandatory and statutory essential training skills, for example, Basic Life Support, manual handling, infection control, fire etc
- Education and training to NVQ level 3/Work based diploma or equivalent
- 5 GCSE grade 4 or C and above or equivalent
Experience
Essential criteria
- Relevant experience of working in Health or Social Care Environment delivering personal care
Desirable criteria
- Relevant experience working in the NHS
Skills / Knowledge / Abilities
Essential criteria
- Ability to work under instruction from a Registered Professional without direct supervision and support
- Digital Skills, use of systems and ability to use physiological recording equipment and record findings
- Good communication skills and ability to document care episode or event on digital systems or written
Desirable criteria
- Evidence of development of knowledge and skills relevant to role
- Training and competence in clinical tasks relevant to role ie. Cannulation, wound care, ECG

- Full driving licence and access to a vehicle (for domiciliary community role)
