It’s an exciting time for our Hospital@Home Service. We are expanding and recruiting to our new teams who work with patients in their homes to provide a virtual ward service for early supported discharge and admission avoidance. 

Hospital @ Home teams provide specialty virtual ward beds. They will be multi-speciality supporting patients across respiratory, heart failure and frailty in partnership with acute care, consultants and specialist nurses.   

We are looking for Senior Staff Nurses to join us, so if you’re an ambitious, proactive, and experienced nurse looking to be part of a supportive team and use your skills to provide excellent community care, we’d love to hear from you.  

 This is an amazing opportunity to become a member of the team as a senior staff nurse working together to support patients through the critical first few days after they leave hospital and avoid unnecessary hospital admissions. The focus of this role is delivering holistic assessments, planning, implementing, and evaluating treatment care plans, alongside reviewing care provision. 

 We are a multidisciplinary team working flexibly to provide a seven-day service, 365 days a year, between 8am and 8pm. 

The team works closely with other services within Sussex Community NHS Foundation Trust, as well as our acute and community partners, GPs, South East Coast Ambulance Service (SECAmb), adult social care and voluntary and third sector organisations. 

Main duties of the job

In this role, you will use your clinical expertise and problem-solving skills to support patients and the team. This involves completing clinical assessment, planning and the implementing care and identifying deteriorating patients.  

As a member of the team, you will receive support from members of the multidisciplinary team and participate in the in-service learning programme. While developing your own specialist areas of practice, relevant to the team, you will supervise and support the development of colleagues, including those outside of your professional group and pre-registration students. 

You will play an active role in the development of the service through reflective practice, clinical supervision, and quality improvement initiatives. This is a fantastic environment in which to develop your leadership skills through formal and supported training, using these to assist in shaping future clinical practice of the team. 

 You will need to able to drive with a full UK licence  and access to a car due to the geographic area covered by the team.

  • Evidence of post registration development

Experience

Essential criteria
  • Experience as a Band 5 Nurse working in the community.
  • Demonstrates commitment to and recent evidence of further education and professional development
  • Experience of supervising, coaching/supporting/mentoring less experienced staff
  • Experience of individual performance review
  • Experience of holistic patient assessments
Desirable criteria
  • Experience of the Extended Role in cannulation and Intravenous therapy administration
  • Competent in venepuncture and taking ECG

Skills and Knowledge

Essential criteria
  • Able to demonstrate organisational and time management skills.
