Job overview
We are currently in the process of relocating Elective Colorectal & Lower GI Cancer Surgery and Stoma Reversal Surgery from Royal Sussex County in Brighton to the Worthing site, creating a centre of excellence for Colorectal Cancer Surgery delivered across at Worthing and St Richard’s hospitals
This is an exciting development where we be investing in new theatre and bed capacity and associated surgeon, anaesthetic, nursing, therapies and other workforce requirements to meet the additional demand in Worthing.
This new post will be key in supporting the delivery of the service.
The post holder will work with the Stoma Care Nursing team in delivering and managing an expert Stoma Care Nursing Service for University Hospital Sussex NHS Foundation Trust predominantly in the community and at Worthing General Hospital. The role involves patient centred individualised care to those undergoing Colorectal, Urology and Gynaecology surgery resulting in a stoma. You will also support paediatric and neonatal stoma care services within both secondary and primary care settings. You will provide professional and clinical expertise to assist the physical, psychological, and social rehabilitation of patients requiring surgery involving stoma formation.
Main duties of the job
You will provide specialist services and advice to patients and their families who potentially or require stoma forming surgery. This role will require the post holder to undertake community visits and will run nurse-led clinics and will teach a wide range of healthcare professionals and will be able to lead the team at times and support and direct the junior stoma care CNS’s. The position would be suitable for a registered nurse with proven experience working as a Stoma Care CNS with Surgical and Colorectal experience. You will be looking to further develop your career as a Stoma Care Nurse specialist and will have a passion for the speciality and service development.

Detailed job description and main responsibilities
Patient Care Delivery
- Predominantly community-based role but will be expected to work within Worthing hospital site as required, depending on workload or to cover leave.
- To independently maintain and organise own case load ensuring high standards of appropriate information giving and care both physical and psychological.
- Accept referrals for and organise to see elective patients and carers pre-operatively to provide information and education with regards to their proposed surgery.
- Site elective patients for their stoma forming surgery or possible stoma forming surgery and where possible patients having emergency surgery involving the patient/carer in decision making process.
- Identify educational needs of individual patients and their carer’s and facilitates the meeting of those needs to ensure timely discharge.
- Participate in maintaining the Stoma Care Helplines.
- To lead ward staff in developing a plan of care for each patient with discharge planning central to this process.
- Provide samples and advice on appliances and accessories as required, ensuring impartiality at all times.
- To provide follow up care and support during home visits and in nurse led outpatients clinics.
- Undertake autonomous Nurse Led Stoma Care Clinics seeing both new and follow up patients where plans of a care and treatment decisions are made and implemented.
- To maintain accurate documentation and provide verbal and written reports as required ensuring patients are not lost to follow up.
- Act as an expert in complex cases to the MDT, communicating relevant information and ensuring appropriate referral and management for these patients.
- To support senior Stoma Care Nurse Specialists to see groups of patients including paediatrics, neonatal and those who have internal pouches or require colostomy irrigation training.
Communication
- Ensure and promote excellent communication with all members of the MDT.
- Provide condition related information to patients in clinic setting and at the bedside (pre-op and post-op), to include gaining informed consent, requiring empathy and reassurance.
- Lead once-monthly patient groups within the community, providing information, support etc.
- Attend weekly clinical meetings as needed to foster MDT working.
- Ensure confidentiality and adherence to data protection legislation.
- Ensure that a full and accurate record of care is documented in a timely manner utilising appropriate nursing and medical systems as well as our bespoke stoma care database.
- Ensure stoma care database and diary are used effectively and accurately.
- Promote inclusiveness and treat people with kindness and compassion.
- Promote a team ethos within the stoma care department and promote a positive working environment.
- Ensure grievances/disputes/complaints are investigated thoroughly in a timely manner.
