Are you looking for an exciting and varied job providing rehabilitation to patients in the community following Stroke? As a Generic Associate practitioner, training will be offered to develop your abilities within Physiotherapy, Occupational Therapy and Speech therapy and Nursing provision within Stroke An opportunity has become available for a motivated individual to join the Hull and East Riding Stroke Service as a Generic Associate Practitioner. This service provides specialist assessment and rehabilitation to Stroke Survivors living in the Hull and East Riding area. We offer rehabilitation in a bedded rehab unit and in the community in the patient’s own home. 

Main duties of the job

You will be responsible for delivering individual rehabilitation programmes using a goal setting approach. You will have previous relevant experience of working within a rehabilitation setting. Currently there is a need for Associate Practitioner support within the East Riding locality. The Hull and East Riding Community Stroke Service has a strong focus on clinical governance and provides a supportive framework for further learning including Stroke based competencies.

This post is key to the performance of the team, and your drive and enthusiasm will help support the success of the service. You will need effective interpersonal skills and be flexible in your approach.

Qualifications

Essential criteria
  • Relevant NVQ level 4 or Foundation Degree in Health and Social Care or demonstrable working experience in the field of rehabilitation.

Knowledge

Essential criteria
  • Demonstrate awareness of healthy living practices
  • Knowledge of the Health and Safety at Work Act
  • Knowledge of preparing rehabilitation programmes for improving independence and fitness.
  • Knowledge of risks associated with performing exercise and when to escalate clinical concerns to the most appropriate clinician.
  • Clear understanding of the rehabilitative process and role to play within this.
Desirable criteria
  • Knowledge of the Mental Capacity Act, Safeguarding Adults and Young People

Experience

Essential criteria
  • Experience of working within a clinical care setting
  • Experience of engaging people in healthy living practices.
  • Experience of working with groups of people.
  • Experience of working in a multi-disciplinary environment.
Desirable criteria
  • Evidence of working within rehabilitation clinical services
  • Previous experience working with people with physical needs and adults with complex needs
  • Experience of working with people with chronic/progressive disease
  • Experience of liaising and working with multidisciplinary teams or care teams
