An exciting opportunity has arisen for a motivated, compassionate, and enthusiastic Band 6 Nurse to join our Hull & East Riding Adult Community Team for Learning Disabilities.

In this role, you will work collaboratively within a highly skilled multidisciplinary team to address the health needs of adults with learning disabilities and reduce the health inequalities they face. You will play a key role in delivering high‑quality, person‑centred care that promotes independence, wellbeing, and meaningful inclusion in the community.

A significant part of this role involves leading and coordinating the Complex Care Team, which provides specialist support to adults with profound and multiple learning disabilities living in the community. This includes holding a caseload of individuals with complex health needs, ensuring their care is regularly reviewed, monitored, and adapted in line with best practice.

You will oversee the day‑to‑day functioning of the team, provide clinical and managerial supervision to staff, and ensure safe, effective, and compassionate care is consistently delivered.

Main duties of the job

As a Band 6 Nurse, you will take on a key leadership role within the wider service.

You will act as a clinical role model, supporting colleagues through guidance, supervision, and mentorship, and contributing to a positive, inclusive team culture.

You will be expected to demonstrate strong professional judgement, lead on complex clinical situations and hold a caseload of clients with more complex needs, and support junior staff in developing their skills and confidence.

You will also contribute to service development and quality improvement initiatives, helping to shape and strengthen pathways and embed best practice across the team.

You will undertake comprehensive assessments, develop and implement personalised care plans, and work closely with individuals, families, and carers to promote physical and mental health.

You will support people to overcome barriers to accessing healthcare, advocate for reasonable adjustments, and contribute to reducing health inequalities across the region.

Your role will also involve close partnership working with primary care, acute services, social care, and voluntary sector organisations to ensure coordinated, holistic support.
