Job overview
Humber Teaching NHS Foundation Trust’s Award winning YOURhealth service are looking for an enthusiastic, dedicated individual who is passionate about helping and supporting people to change their lifestyle as a Health and Wellbeing Advisor and Connector within the YOURhealths Social Prescribing service.
The post advertised will be to work with our existing team of dedicated Social Prescribers in several locations in the East Riding, namely Bridlington, Goole and Withernsea. This particular role will be based in Goole and the surrounding villages with one day a week potentially in Market Weighton but may require travel to other locations within the East Riding at times.
Health & Wellbeing, Advisor & Connector workers are there to offer individualised, personal support and motivation to adults who may require personalised solutions to social, emotional or practical needs that will empower each individual person accessing the service to empower self, leading to improvements in their health and wellbeing.
Thursdays and Fridays will be required to work in Goole 9am - 5pm, 1 day may be required in Market Weighton and a day potentially to work from home depending on needs of the service.
Main duties of the job
You will also be expected to promote and market healthy lifestyle issues including social health within the local communities to encourage, change within areas of deprivation and isolation.
Excellent interpersonal communication skills are an essential requirement of Health and Wellbeing Advisor and Connectors as they will be working with individuals (and occasionally groups of people) from a variety of backgrounds and have a sound understanding of behaviour change techniques and asset based community development.
The post holder must be flexible to work daytime and evenings, including weekends. You must also be required to commute effectively across the geographical boundaries of East Yorkshire.

Qualifications
Essential criteria
- A good understanding of physical to Foundation Degree equivalent to diploma level including health promotion, and mental health including policy/legislation
Knowledge
Essential criteria
- Working knowledge of behaviour change, emotional wellbeing and key public health issues
- Excellent IT skills.
Experience
Essential criteria
- Demonstrable experience of working within community service settings, areas of isolation, rurality and deprivation.
- Has an understanding of health needs and issues across East Riding
Desirable criteria
- Minimum 1 year facilitation/coaching experience in mental health, social health and physical health
Skills and competencies
Essential criteria
- Manage own time and work on their own
- Ability to motivate others
- Evidence of effective inter-personal communication skills
