We cover the whole of Hull and East Riding so the ability to commute within CHCP CIC's geographical areas with access to appropriate means of transport is essential.

Prif ddyletswyddau'r swydd
- Assist with the implementation of clinical and social care packages in people’s homes
- Provide personal care, including washing, dressing and meal preparation
- Support with safe medication administration
- Build positive relationships with patients, families and colleagues
- Work a range of shifts including 08:00–14:00, 08:00–16:00, 14:00–22:00 and 16:00–22:00, across a seven‑day service including bank holidays
- Experience delivering personal care in a health or social care setting
- Confidence working in people’s homes and supporting varied needs
- Good communication skills and a caring, reliable approach
- A willingness to learn, with training and support provided

Qualifications
Meini prawf hanfodol
- Level 3 in Health and Social Care or equivalent level of experience in Health and/or Social Care
- Medication administration training or willing to undertake this as part of the induction period
- Care Certificate or willingness to complete this within a 6 month induction period
Knowledge & Experience

- Experience of working in a health and/ or care environment
- Experience of prioritising own work load
- Good understanding of basic IT skills

- Knowledge and experience of administering medication
