Job overview

You’ll support unpaid carers through personalised assessments and practical help, making it easier for them to manage their caring role and stay well.
 
As part of the Carers Information and Support Service, you’ll work closely with colleagues across the “hub and spoke” model to make sure carers can access the right support at the right time. You’ll build trusting relationships, communicate with compassion, and help carers navigate complex situations with confidence.

Main duties of the job

What you’ll do:
  • Carry out Carer Assessments and agree support plans tailored to individual needs
  • Hold and manage your own caseload, keeping clear and accurate records
  • Work with partners and care providers to ensure coordinated support
  • Provide information, signposting and referrals, including Direct Payments where appropriate
  • Take part in service development, training and promotional activity
What we’re looking for:
  • Level 3 qualification in Health and Social Care or equivalent knowledge and experience
  • Strong communication skills, including the ability to explain sensitive information
  • Ability to work flexibly, manage your own workload and respond to changing needs
  • Confidence using IT systems such as electronic records
  • A positive, inclusive approach and a commitment to the Seven Cs values

Qualifications

Essential criteria
  • Level 3 in Health and Social Care or able to demonstrate equivalent level of knowledge, skills and experience
Desirable criteria
  • Communication qualification or undertaken a communication skills module

Knowledge

Essential criteria
  • Able to demonstrate a full understanding and knowledge of carer issues, including demonstrable experience of carrying out carer assessments
  • Ability to develop effective working relationships with multi-professional teams
  • Grounded in evidence based practice
  • Awareness of National and Regional developments for carers
  • Have a good range of IT skills, including Microsoft Office
  • Knowledge of DDA, Benefits and Carers Rights
Desirable criteria
  • Knowledge of local and national networks which impact upon Carers

Experience

Essential criteria
  • Experience of undertaking comprehensive assessments
  • Experience in a social and/or health care setting
Desirable criteria
  • Evidence of continual professional development
  • Experience of working within a community setting
  • Practical experience of researching information
