Job overview
We are looking for enthusiastic an enthusiastic and dynamic Registered Health Care Professional to join a well-established Trusted Assessor Team within Hull and East Riding Intermediate Care. The role requires the post holders to competently assess patient’s suitability for rehabilitation following inpatient hospital treatment. The role also involves the assessment of patients who may not be managing in their home environment and require rehabilitation within a bedded unit facility.
There is the requirement to work between CHCP Bases as required and in reach into Hull Royal Infirmary and Castle Hill Hospital. As a Trusted Assessor you will play a vital role into the Intermediate care model. This involves closely working with system partners to optimise patient care.
Main duties of the job
The successful applicant will be part of a supportive multi-disciplinary team and will receive regular supervision, the Intermediate Care team is proactive in supporting development opportunities.
The successful candidate will have a broad knowledge base, good time management skills and be able to demonstrate relevant experience for this role. You should have an excellent communication skills and ability to work collaboratively with staff from all disciplines whilst maintaining high professional standards.
Trusted Assessor within Intermediate Care is a 7 day, 8am to 8pm service, therefore the post holder will be required to participate in the weekend Rota.
The successful applicant will be required to travel between the community bedded units and patient’s homes in and around Hull and should therefore have access to a suitable means of transport.

Qualifications
Essential criteria
- Degree in Physiotherapy, occupational Therapy or nursing equivalent
- Current full HCPC/NMC Registration
- Post-graduate diploma in relevant area
Desirable criteria
- Member of special interest group
Knowledge
Essential criteria
- Able to justify clinical decisions to other Medical Staff, colleagues and other professionals
- Competent in skills appropriate to the post e.g. Carry out clinical assessment of patient need in line with intermediate care team requirements
- Evidence of CPD activity, including nationally recognised post graduate training in relevant speciality to Diploma level or equivalent
- Evidence of reflective practice, IST and CPD portfolio
- Autonomous practitioner works within codes of practice, policies and guidelines.
- Accountable for own professional action
- Contributes to the development of policy.
Desirable criteria
- Evidence with involvement with clinical governance agenda
Experience
Essential criteria
- Broad based clinical experience covering main speciality areas including community work in relevant specialism.
- Able to work alone/unsupervised.
- Ability to supervise more junior staff and support staff.
- Good Time management.
- Ability to manage own caseload.
- Able to work in a MD/Multi-Agency setting
Desirable criteria
- Participation in audit/evaluation/research
- Clinical experience within specialist field
