HULL and EAST RIDING UTC’s City Health Care Partnership CIC is seeking highly motivated and enthusiastic practitioners to join our team in Urgent Treatment Centres across Hull and East Riding of Yorkshire. This position offers a two-day-per-week working pattern, ideal for flexible working. The practitioner will be responsible for the clinical assessment of patients, including history-taking, clinical examination, diagnosis and treatment or the onward referral of patients of all ages. Urgent and primary care patient consultations will be undertaken via face to face, telephone or digital platform consultation. Patient presentations will include undiagnosed undifferentiated illness and minor injuries, acute presentations and at times the exacerbation of long-term conditions. The practitioner will be responsible for ensuring that the highest standard of clinical care is provided for patients across CHCP. The post holder will be a clinician with a range of experience of caring for and treating patients with minor injury and/or minor illness conditions or presentations.

The practitioner will act within their professional boundaries and will support the delivery of policies and procedures. They will act as a positive role model supporting junior staff whist working flexibly between urgent care and primary care to provide safe clinical expert care. Effective leadership skills will be required and the successful candidate will be one who is motivated, dedicated and enthusiastic. The post holder will work collaboratively with partner organisations to ensure that care is seamless and timely to ensure the best possible patient experience. They will also promote exceptional standards of patient care underpinned by the implementation of evidence-based practice, audit and with regard to current available guidelines and research. There will be an expectation to work across all bases within the Hull and East Riding locality. Working as part of an established clinical workforce, they will benefit from a multi-disciplinary team, and be exposed to an extensive range of services and healthcare settings and locations. They will work alongside our newly developed multi-professional clinical model, be supported by a network of clinicians to share expertise.

Qualifications

  • Professional registration with the NMC (Nursing and Midwifery Council) or the HCPC (Health and Care Professions Council).
  • Approved degree in nursing or paramedic science or equivalent experience.
  • Post graduate qualification in relevant subject or equivalent experience
  • Teaching/Mentoring qualification
  • APACS (Advanced Physical Assessment and Consultation Skills) or equivalent qualification/experience
  • Non-medical prescribing qualification

Knowledge

  • Highly specialised knowledge of relevant policy and legislation in relation to unplanned care
  • Working knowledge of multi-disciplinary teams
  • Evidence of Continuing Professional Development
  • Specialist knowledge of medication and interventions used in unplanned care
  • Demonstrate the use of evidence based practice
  • Knowledge of the legal framework surrounding medications prescribed or supplied, including PGDs, PSDs, EOL care pathways etc.

Experience

  • Experience in a specialist environment, demonstrating effective leadership and autonomy. Previous experience of working in a similar post
  • Experience of multi-disciplinary team working and working with patients with minor injuries and minor illness presentations
  • Experience of the initial management, stabilisation and onward referral of patients who may self-present with more serious/life threatening conditions
  • Previous experience of producing reports or audits
  • Experience of utilising a variety of software packages e.g. SystmOne, and E-Mail,
  • Ability to prioritise effectively in a clinical environment
  • Previous leadership and management experience within a clinical setting
  • Experience and competency in supervision and support of others including the prioritisation of patient care e.g. junior staff, non-registered staff and students
