Senior Case Manager (Older Adult Mental Health)

SECONDMENT or FIXED TERM (6 months )

An exciting opportunity has arisen for a Senior Case Manager (Older Adult Mental Health) at the  Humber and North Yorkshire Mental Health, Learning Disability and Autism  Collaborative.

The post holder will work alongside the Mental Health, Learning Disability and Autism Collaborative to provide additional centralised capacity to drive the Out of Area Hospital placements work. 

Please ensure approval for secondment is sought prior to application being submitted. This is a part-time post of 22.5 hours (3 days per week).

The post holder will work as part of a dynamic team in delivering an effective service; supporting managers and staff across Humber and North Yorkshire to ensure safe and effective care pathways are in place for service users of specialised mental health care settings within the NHS and Independent sectors.

This will include:

  • Undertaking face to face quality visits – which will include travel outside of the Humber and North Yorkshire region.
  • Agreeing robust discharge plans in line with national guidance
  • Liaising with key partners from the system to ensure local services can meet individual need, and oversee the repatriation of individuals. Partners will include named local case manager, social care, housing, complex care directorate, local NHS providers, specialised commissioning, MoJ

  • Ability to negotiate on difficult and controversial issues including performance and change.

Qualifications & Knowledge

  • Registered Mental Health / Learning Disability Nurse or other registered health or social care professional
  • Educated to master’s level or equivalent level of experience of working at a senior level in specialist area.
  • Knowledge of Quality improvement tools/processes e.g., clinical audit, investigation tools, national serious incident policy
  • Knowledge and experience of one or more specialist areas, acquired through post graduate diploma or equivalent experience or training

  • Have an appreciation of the relationship between the Department of Health and individual provider and commissioning organisations.
