Job overview

We are looking for a full-time pharmacist to join Humber Teaching NHS Foundation Trust.

The post holder will be a pharmacist, who acts within their professional boundaries, supporting and working alongside a team of pharmacists in general practice. In this role they will be supported by a Specialist Clinical Pharmacist who will develop, manage, and mentor them. 

The post holder will work as part of a multi-disciplinary team in a patient-facing role across the trust. 

 The post holder will provide primary support to other health care professional with regards to prescription and medication queries.

They will help support the repeat prescription system, deal with acute prescription requests, and medicines reconciliation on transfer of care  for safer prescribing, providing expertise in clinical medicines advice while addressing both public and social care needs of patient in the different sectors of the trust 

The post holder will provide clinical leadership on medicines optimisation and quality improvement.

The role is pivotal to improving the quality of care and operational efficiencies so requires motivation and passion to deliver excellent service within the trust.

We are willing to consider relocation package to the successful candidate if appropriate. 

Main duties of the job

This will be a rotational post, primarily of a training and development nature. Levels of responsibility will increase as experience and competence increases.

The post holder may be offered the option of undertaking a Clinical Pharmacy Diploma, or an alternative but broadly equivalent post-registration training programme in accordance with NHS England training recommendations for pharmacists.

The post holder will progress to a band 7 post on completion of appropriate training requirements .

 Any alternative training requirement would be specified and agreed by applicants prior to them accepting the post.

This is a Band 6-7 transitional role and progression to band 7 is  dependent on the successful completion of a portfolio of evidence aligned to the foundation level framework published by the Royal Pharmaceutical Society (RPS).

You will be expected to act in accordance with the General Pharmaceutical Council’s GPhC standards for pharmacy professionals.

Knowledge and experience
