Main duties of the job

What you’ll do:
  • Carry out holistic assessments and plan individualised care
  • Deliver nursing interventions such as wound care, medication administration and health education
  • Communicate effectively with patients, families and partner agencies
  • Maintain accurate records in line with professional standards
  • Contribute to service improvements and uphold CHCP policies
What we’re looking for:
  • Registered Nurse (or working towards final assessment)
  • Understanding of community nursing and evidence-based practice
  • Ability to work independently and as part of a team
  • Basic IT skills and willingness to learn
  • Flexible approach to meet service needs, including occasional weekends and bank holidays

Qualifications

Essential criteria
  • Degree in Nursing Studies / First level Registered Nurse or working towards final assessment to achieve first level registration
Desirable criteria
  • Post-registration qualifications
  • Mentorship qualification

Knowledge

Essential criteria
  • Demonstrate an understanding of the role of a community nurse
  • Knowledge of legislation impacting on service delivery
  • Knowledge of the modernisation agenda
  • Demonstrate an understanding of evidence based practice
  • Knowledge of local initiatives
  • Demonstrate an understanding of the MDT
  • Basic IT skills
  • Knowledge and skills in the assessment process
Desirable criteria
  • Able to mentor students
  • Evidence of professional development
  • Have a working knowledge and understanding of First Visit Criteria

Experience

Essential criteria
  • Basic health and safety awareness
  • Experience of working within a healthcare setting
  • Experience of working with families and carers
  • Experience of working in a team
  • Experience of working with computerised recording systems
Desirable criteria
  • Experience of multi-agency working
  • Experience of working within a community setting
  • To have recently worked within a nursing capacity
  • To be able to provide education to patients, carers, staff and other health care professionals
