Job overview
Main duties of the job
- Carry out holistic assessments and plan individualised care
- Deliver nursing interventions such as wound care, medication administration and health education
- Communicate effectively with patients, families and partner agencies
- Maintain accurate records in line with professional standards
- Contribute to service improvements and uphold CHCP policies
- Registered Nurse (or working towards final assessment)
- Understanding of community nursing and evidence-based practice
- Ability to work independently and as part of a team
- Basic IT skills and willingness to learn
- Flexible approach to meet service needs, including occasional weekends and bank holidays

Essential criteria
- Degree in Nursing Studies / First level Registered Nurse or working towards final assessment to achieve first level registration
Desirable criteria
- Post-registration qualifications
- Mentorship qualification
Knowledge
Essential criteria
- Demonstrate an understanding of the role of a community nurse
- Knowledge of legislation impacting on service delivery
- Knowledge of the modernisation agenda
- Demonstrate an understanding of evidence based practice
- Knowledge of local initiatives
- Demonstrate an understanding of the MDT
- Basic IT skills
- Knowledge and skills in the assessment process
Desirable criteria
- Able to mentor students
- Evidence of professional development
- Have a working knowledge and understanding of First Visit Criteria
Experience
Essential criteria
- Basic health and safety awareness
- Experience of working within a healthcare setting
- Experience of working with families and carers
- Experience of working in a team
- Experience of working with computerised recording systems

- Experience of multi-agency working
- Experience of working within a community setting
- To have recently worked within a nursing capacity
- To be able to provide education to patients, carers, staff and other health care professionals
