Job overview
We are looking for a motivated and compassionate Band 5 Community Mental Health Nurse to join our Primary Care Mental Health Network (PCMHN) team covering the Harthill and River & Wolds localities.
This is an exciting opportunity to work at the front end of community mental health services, supporting individuals with mild to moderate mental health needs within primary care settings.
About the role
You will play a key role in delivering high-quality, person-centred care, including:

Undertaking comprehensive mental health assessments and risk assessments

Developing and reviewing personalised care plans

Delivering low-level, evidence-based interventions including structured clinical management and guided self-help

Providing trauma-informed care, including stabilisation work
-
Supporting individuals to build coping strategies and improve wellbeing
You will work as part of a supportive multidisciplinary team, with strong links across Humber services including PCMHNs, CMHTs, Crisis Teams, and Talking Therapies.
The role also involves partnership working with external services such as Addiction services, Evolve (eating disorders), and Autism services, including joint working and shared care planning.
Main duties of the job

To undertake comprehensive mental health assessments, including risk assessment, within a primary care setting.

To develop, implement, and review person-centred care plans in collaboration with service users, carers, and the wider multidisciplinary team.

To deliver low-level, evidence-based interventions, including structured clinical management, guided self-help, psychoeducation, and trauma-informed stabilisation work.

To provide trauma-informed care that is safe, compassionate, and responsive to individual needs.

To support individuals to develop coping strategies, emotional regulation skills, and improve overall mental wellbeing.

To work collaboratively within the Primary Care Mental Health Network and wider Humber services, including CMHTs, Crisis Teams, and Talking Therapies.

To engage in partnership working with external services such as Addiction services, Evolve (eating disorders), and Autism services, including joint working and shared care planning.

To participate in multidisciplinary team meetings, case discussions, and joint assessments to ensure coordinated and effective care.

To manage a defined caseload, prioritising workload effectively and working both independently and as part of a team.

To work flexibly across a range of settings, including GP practices, community venues, and remote/home working.

To travel across the East Riding area as required to meet service needs.

Qualifications and Knowledge
Essential criteria
- Post registration/graduate diploma or equivalent experience in related work area, to demonstrate advanced level of practice
- Full understanding of relevant policy, drivers and their application to clinical and service area.
Desirable criteria
- Evidence of CPD or experiential learning at an advanced level e.g. Post graduate/Masters level
Experience
Essential criteria
- Evidence of continuing professional development (CPD) relevant to the clinical area at specialist level of care
- Evidence of promoting/supporting active user/carer involvement/participation
Desirable criteria
- Development of specialist/advanced level of practice skills and able to demonstrate the impact of this on practice change/development
Skills and Competencies
Essential criteria
- Able to demonstrate effective communication skills
- Ability to demonstrate ethical values and attitudes within a culture of equality and diversity
