Job overview

We are looking for an enthusiastic, confident experienced Renal nurse to join our team as a band 6 home therapies nurse. You should have wide ranging experience in renal nursing and should be able to draw on knowledge and skills in coordinating the care of patients who choose to have dialysis in their home environment.

You will be required to provide excellent nursing care, utilizing the specialist knowledge to teach and educate patients and carers undergoing home dialysis. You will work collaboratively with the wider renal multi-disciplinary team and support the renal ward with staff training and clinical expertise. The postholder will be responsible for the assessment, training, and coordination of proposed and existing home therapy patients. Working alongside the Advanced Kidney care team educating on different dialysis options. Further Dialysis training will be provided as required.

The role requires flexibility and includes travelling to visit patients in their homes as part of their care and follow up.  Access to a car and the possession of a UK driving license is essential to this post.

The Renal home therapies department is operational between 08:00 and 17:00.

Main duties of the job

  • To teach patients how to carry out their own dialysis treatment at home and to provide specialist nursing care and support to patients and their carers 
  • Educate patients /provide counselling on different treatment options for patients with  Advanced Kidney diseases
    • To be  responsibility for the care of a caseload of patients requiring specialist nursing care within the hospital and home setting.
  • To work collaboratively  and coordinate the development of home dialysis unit making best use of available resources.
  • To provide an effective and responsive service to patients and carers within the trust and in their homes.
  • To provide practice development support to renal nurses.
  • Participate in the procurement of dialysis consumables and the home delivery services as appropriate.
