Are you a newly qualified Physiotherapist, or a student soon to qualify, looking for an exciting opportunity to launch your career in a supportive and rewarding environment?

We are looking for an enthusiastic and motivated Physiotherapist to join our Community Therapy Team, This is an excellent opportunity to develop your clinical skills while supporting patients to achieve their rehabilitation goals within their own homes and local community settings. 

Community Therapy offers a unique and rewarding experience, allowing you to work closely with patients throughout their rehabilitation journey, promoting independence, recovery and quality of life. 

You will work as part of a multidisciplinary team assessing and treating adults with a wide range of conditions and supporting them to regain function and confidence in their daily lives. 

You will benefit from regular supervision, a structured preceptorship programme, and ongoing  development opportunities designed to build confidence, competence and clinical expertise.

Prif ddyletswyddau'r swydd

Provide a high quality Physiotherapy service to patients referred to the Community Therapy team. 

Hold clinical responsibility for a designated caseload of patients.

Undertake a high standard of Physiotherapy assessments of patients with diverse presentations using clinical reasoning skills to provide a diagnosis and deliver individualised treatment programmes.

Provide advice and education to optimise health, wellbeing and to promote better self-management of a condition.

Work as an autonomous practitioner within the multidisciplinary team.

Ensure timely discharge of patients from the service and refer patients on to other health, social care and voluntary agencies as appropriate.
