Cardiac Associate Nurse Specialist - Cardiac Rehabilitation

Mid and South Essex NHS Foundation Trust is the only acute hospital provider in the Mid and South Essex Integrated Care System (ICS), which is a system of health and care partners working together for our local population.
Within Mid and South Essex there are 149 GP practices, operating from over 200 sites, forming 27 Primary Care Networks and one Ambulance Trust. There are also three main community and mental health service providers who work together through a community collaborative.

Job overview
Our Cardiac Rehabilitation team at Broomfield hospital, made up of a multidisciplinary team which complements each other in the care and provision of cardiac rehabilitation services for our patients, are looking for a Cardiac Nurse Specialist to join its close knit and supportive team.
Covering the whole of Mid Essex, we hold Cardiac Rehabilitation classes both in the hospital as well as in local gyms and halls in Maldon, Braintree and Witham.
As our Cardiac Nurse Specialist, you will be responsible for supporting the current cardiac rehabilitation service, its on-going management and future development, supporting and co-ordinate an effective high quality Cardiac Rehabilitation service that includes physical, educational, emotional and psychological support of patients, relatives and carers enabling them to live with their disease, treatment and the implications thereafter.
Main duties of the job
In your role you will be involved in all phases of the cardiac rehabilitation process, specialist nurse led clinics and CPET service, participating in the monitoring and improving standards of care through supervision of practice, clinical audit, developing practice, contributing to research, teaching and supporting professional colleagues, liaising effectively to establish and maintain professional relationships with the multi-disciplinary team, agencies and organisations for both inpatients and outpatients and will be working across sites (Broomfield/ Maldon/ Braintree districts).
Your role will be to act as an information resource for patients, carers and staff, establishing good relationships with patients and their families through all phases of cardiac rehabilitation, facilitating Phase II telephone follow up calls and ensure accurate documentation and to support a patient case load for the Cardiac Rehabilitation exercise phase III programme and take responsibility for the patients care and discharge.
You will assess, plan, deliver and evaluate patients care utilising an individual holistic approach, monitor all relevant Nice Guidelines and make changes as appropriate and carry responsibility for the planning of discharge from rehabilitation of cardiac patients including future goal setting, implementation of secondary prevention issues and informing primary care clinicians of patient’s status and future needs.

- Registered Nurse with NMC
- Recognised professional cardiology/ cardiac rehabilitation qualification
- Teaching course or equivalent
Desirable criteria
- Recognised exercise qualification e.g BACPR
Experience

- Minimum 3 years experience in a cardiac post
- Understanding of the patients cardiac pathway
Desirable criteria
- Cardiac rehabilitation experience
