Clinical Nurse Specialist – Heart Failure

We have an exciting opportunity for an experienced and motivated professional to join our dedicated Heart Failure team as a Clinical Nurse Specialist. In this pivotal role, you will have ongoing responsibility for delivering a high-quality, evidence-based service for patients admitted with heart failure, ensuring they follow the right care pathway and helping to reduce hospital length of stay.

You will play a key part in shaping clinical practice, delivering both national and local priorities, and leading education, training, and audit initiatives. Empowered to make advanced clinical decisions, you’ll provide expert advice and support to colleagues, working closely with the multi-disciplinary team to ensure the best possible outcomes for patients.

Collaboration is at the heart of this role — you’ll work in partnership with patients, their families, and healthcare professionals across all areas, ensuring a seamless, compassionate, and effective service.

Main duties of the job

  • Promote excellent practice trust-wide in the assessment, diagnosis and management of patients who develop, or are admitted with HF, practising autonomy as a core member of the cardiac team and wider MDT to reduce Length of Stay.
  • Act as a liaison between the Acute Medical Team, Cardiologists, Nurses and MDT ensuring timely review of HF patients, including advice, and requesting of specific tests and appointments.
  • Participate in setting up and running nurse led HF clinics.
  • Prescribe appropriate medications as a non-medical prescriber, and adjust/advise on treatment changes, continually evaluating the quality of patients care.
