Specialist Nurse - Diabetes

Mid and South Essex NHS Foundation Trust is the only acute hospital provider in the Mid and South Essex Integrated Care System (ICS), which is a system of health and care partners working together for our local population.

Job overview

We are seeking to appoint a Band 6/7 (depending on experience) Diabetes Specialist Nurse on a permanent part time (18.75 hours - 0.5 WTE) basis. This role will be based at our Broomfield Hospital site.

You will work as part of specialist MDT team to provide a specialised diabetes service for hospital in-patients and outpatients supporting a caseloads of people with diabetes, utilising technologies such as continuous glucose monitoring, insulin pump therapy and Hybrid closed  loop systems.

You will provide clinical advice and support through education, training and health promotion for people with diabetes and their families / carers. Act as resource for other HCPs  such as  practice nurses, Doctors, the ICT, ward clinical staff, in relation to the development of clinical management plans for people with diabetes.

Main duties of the job

To educate and empower people with diabetes to self -manage, working in collaboration with the other DSNs, consultants and HCPs

Assess, plan and implement nursing care for a caseload of people with diabetes, working in collaboration with consultant colleagues

Provide a high level of communication by liaising with all appropriate members of the multi-disciplinary team.

Work within the remit of non-medical prescribing

Obtain appropriate consent for all procedures.

Challenges inappropriate practices which impact on the patient/ others/Trust.

Maintains an environment which actively and consistently recognises the patient’s rights including the right to confidentiality in line with the Data Protection Act and GDPR

Committed to sharing good practice across the Trust.

Communicates sensitive information to patients and relatives as appropriate.

Able to use and teach others how to use a wide range of specialist equipment such as continuous blood glucose monitoring equipment.
