Job overview

We are looking for an experienced, motivated clinician to join our team. You will be a registered Occupational Therapist/ Senior Physiotherapist with demonstrable expertise in discharge planning, supporting patients to become experts in their own care and to remain in their own homes.

It is expected that you will have excellent communication skills, enjoy working with a variety of service users of different age groups, have good self-management skills and an ability to prioritise your case load. You should ideally have experience of working in a variety of clinical settings with sound medical, respiratory and social knowledge. 

This post has 7 day working, in a 4 week pattern, with x2 days off prior to or after the weekend. Periods of requiring you to work 10:00-18:00 Mon- Friday once every 8 weeks

Main duties of the job

The key objectives for you in this post are: You are able to provide rapid assessments in a fast-paced environment. You can formulate robust discharge plans. You will need to have excellent communication skills as you will be working with the wider MDT to problem solve and support patients into their preferred place of care.

As a Specialist Therapist you will have the opportunity to undertake a leadership role in planning, coordinating, delivering and evaluating the care provided and be responsible for the supervision of junior staff and students.

You will be able to demonstrate the ability to build networks and relationships over several different clinical teams.  You will embrace innovation and integrated working and will be committed to delivering excellence in care alongside other OTs, Physiotherapists, Associate Practitioners and Assistant Practitioners
