Job overview

This role forms an integral part of the West Essex Care Closer to Home strategy and the Home First approach, both of which are designed to ensure patients receive safe, appropriate care and treatment as close to home as possible.

Working autonomously as an advanced practitioner, the Community Matron will take a lead role within a designated Primary Care Network (PCN), coordinating and delivering proactive, evidence-based care across organisational boundaries. The focus will be on early identification and response to signs of disease exacerbation, ensuring that patients receive timely interventions aligned with current best practice.

The post holder will deliver and coordinate tailored care packages for a high-risk population with complex needs and long-term conditions in the community. The aim is to reduce avoidable hospital admissions and enable safe, supported early discharge.

In addition, the role carries responsibility for making decisions regarding care and service provision across individual patient pathways, spanning primary care, secondary care, and community ward settings.

Main duties of the job

The Community Matron will be a key member of the Integrated Neighbourhood Team, working in partnership with the Primary Care Network (PCN) to deliver a whole-systems approach to the management of health and social care needs. The role will focus on supporting individuals with complex, long-term conditions, whether they are living at home or within a care home setting.

Detailed job description and main responsibilities

The post holder will collaborate closely with general practitioners, the Hospital at Home team, Urgent Community Response, specialist community nursing, therapy services, medicines optimisation teams, adult social care, the voluntary sector, and colleagues across both primary and secondary care. This integrated way of working will ensure that care for patients and their carers is effectively co-ordinated, seamless, and responsive to individual needs.

Educational - Qualifications

Essential criteria
  • RGN or equivalent professional qualification with at least 3 years post registration experience
  • Independent Prescriber or commitment to complete within 1 year of appointment
  • Accredited Consultation & Diagnostics qualification
Desirable criteria
  • Qualification in area of specialist practice

Managerial Experience

Essential criteria
  • Understanding of the wider NHS agenda including the rationale behind the strategy for the management of Long Term Conditions
