Mid and South Essex NHS Foundation Trust is the only acute hospital provider in the Mid and South Essex Integrated Care System (ICS), which is a system of health and care partners working together for our local population.

Within Mid and South Essex there are 149 GP practices, operating from over 200 sites, forming 27 Primary Care Networks and one Ambulance Trust. There are also three main community and mental health service providers who work together through a community collaborative.

We collaborate with other valuable partners, including three Healthwatch organisations, nine voluntary and community sector organisations and three top tier local authorities, with seven district, borough and city councils, as well as our local universities.

 

Job overview

We have an exciting opportunity here at Mid and South Essex Foundation Trust to join our Dementia Service. You will work alongside the Dementia and Delirium Nurse Specialist/Admiral Nurse to support the acute hospital in managing the complex health needs of individuals living with dementia and/or experiencing delirium. Assist in the implementation and evaluation of seamless dementia and delirium care pathways, ensuring patients receive high standards of care throughout their hospital journey and beyond.

The role involves supporting the interface between acute hospital services, primary care, and community-based specialist dementia and delirium services and you will work in partnership with healthcare professionals and multi-agency colleagues, collaborating to meet national, CQC, and local service standards.

Main duties of the job

You will bridge the gap between acute clinical care areas, enabling better communication and improved access to healthcare for individuals with Dementia and/or Delirium.

Actively promote the Dementia and Delirium Service to individuals with Dementia and/or Delirium, their families, carers, and professionals.

Be an advocate for people with Dementia and/or Delirium, ensuring their voices are heard in care planning and service delivery.

Facilitate the meeting of individual healthcare needs for people with complex needs.

Act as an advocate for people with Dementia and/or Delirium To actively assist people with Dementia and/or Delirium, as well as their carer.

Assist in the delivery of training to meet identified needs specifically to ensure that colleagues in acute hospitals are aware of and able to meet the needs of people with Dementia and/or Delirium.

Support working partnerships with primary health care teams, acute hospitals, and other key health care providers across the MSEFT footprint.

Participate with the involvement of the patient in shared decision-making and care planning.

  • Relevant academic qualification DipHE/Foundation Degree/HND/NVQ3
  • Evidence of ongoing professional development

Knowledge and experience

Essential criteria
  • Significant experience of supporting complex patients with Dementia and Delirium
  • Understanding the issues in supporting patients with Dementia and Delirium in acute areas
  • Knowledge of mental capacity act and DOLs
Desirable criteria
  • Experience of working in a multidisciplinary team and carers
  • Experience of dealing with violence and aggression
