Mid and South Essex NHS Foundation Trust is the only acute hospital provider in the Mid and South Essex Integrated Care System (ICS), which is a system of health and care partners working together for our local population.

Within Mid and South Essex there are 149 GP practices, operating from over 200 sites, forming 27 Primary Care Networks and one Ambulance Trust. There are also three main community and mental health service providers who work together through a community collaborative.

We collaborate with other valuable partners, including three Healthwatch organisations, nine voluntary and community sector organisations and three top tier local authorities, with seven district, borough and city councils, as well as our local universities.

 

Job overview

To ensure that an individual programme of care is planned, implemented, and evaluated for each patient within own sphere of responsibility.

To work as part of the multidisciplinary team ensuring the delivery of a high standard of care to the patients.

To act as team leader including taking charge of the ward in the absence of the Ward Manager/Deputy Ward Manager.

To participate in all relevant aspects of the Governance process

The post holder will adhere to the NMC code of Professional Conduct at all times and ensure compliance with hospital policies and procedures.

To act as a positive role model at all times.

Main duties of the job

Make a thorough assessment of the patient’s needs and document these in accordance with Trust policies. Formulate a plan of care based on the identified needs of the patient, taking into account current evidence based research findings. Ensure that all the care planned is delivered to the patient at the highest possible standard. Evaluate and document nursing actions and alter the care plan in accordance with the changing needs of the patient.

To act as an advocate for the patient at all times and promote patient health through education Pro-actively communicate and liaise with all members of the multidisciplinary team on matters affecting patient care. Ensure the rights of patients are respected at all times and assist in the provision of information, enabling informed decisions to be made about their care.

Detailed job description and main responsibilities

In conjunction with the Ward Manager/Deputy Ward Manager, identify and plan appropriate interventions that contribute to high staff morale and good working relationships with all members of the multidisciplinary team. Positively contribute to the process of change within the ward. Contribute to the reduction of healthcare acquired infection by ensuring a high standard of infection control practice in the light of research, current best evidence and legislation. Adhere to the Trust Policies and Guidelines for the Safe Handling, Administration, Storage and Custody of Medicinal Products.
