Job overview

We are looking for an enthusiastic and visionary nurse to join the Transfer of care team at Princess Alexandra Hospital.

You will have a keen interest in complex discharge planning, will have proven communication skills, and be able to work collaboratively with patients, carers and relatives, as well as system partners.

As a team we are committed to enhancing patient experience of discharge planning services which ensure that patients' time and wishes are valued; you will play a key role in these service developments.

Main duties of the job

The Transfer of care Team (TOCT) has representation from Essex and Herts Social Care teams as well as the Discharge Planning team employed directly by Princess Alexandra.

Collectively the TOCT provide expert discharge planning advice and support for patients across the trust who require support from community services on discharge. 

The Transfer of care team are responsible for the management of Head of Transfer of Care Transfer of Care Matron Discharge Facilitator Discharge Support Nurse Patient Journey Co-ordinators 

The main duties of the role will be to plan, provide and maintain effective Discharge Planning care to patients/clients and their relatives.

To continuously develop and enhance knowledge and skills that will directly influence consistent standards of Discharge Planning practice.

Working with the ward based and transfer of care team, support the development of student nurses and junior staff, creating an atmosphere conductive to learning and practice development. 

The role may involve some travel to other hospital sites
