Are you an experienced Senior Nurse or Allied Health Professional (AHP) seeking your next career challenge?

If you are, an exciting opportunity has arisen for a specialised and autonomous practitioner to join our Frailty Interface team as an ACP or trainee ACP (depending on qualifications).

In this development role - you will work towards successfully completing the Advanced Practitioner (AP) MSc (Level 7) programme in line with the Trust framework for advanced practice and their primary professional regulatory body.

You will play a key part in the departments day to day operations by providing high quality care to patients and support to your acute medical colleagues, as well as championing a culture of excellence - enhancing team skills, promoting multidisciplinary collaboration, and driving continuous improvement.

Our Frailty Interface team is multidisciplinary including a Geriatrician, Nurse Consultant, ACPs, tACPs, therapists, Nurses and Nursing associates. The team operates between the Emergency Department and Frailty Assessment Base whilst closely working with our community colleagues in the frailty virtual ward. The Frailty Interface Team support patients living with frailty through comprehensive geriatric assessments, care planning and admission avoidance.

Main duties of the job

During the apprenticeship/training programme you will be supported to provide expert professional and independent clinical care which enables the coordination of a multi-professional seamless service for patients under clinical supervision by a name Educational Supervisor.

Drawing upon a diverse range of knowledge to determine evidence based therapeutic interventions, which will include prescribing medication where legally allowed

You will be expected to demonstrate all four pillars of practice, being involved in and leading upon leadership activities that enable a rich culture of education, research and quality evidence-based practice.

Experience

Essential criteria
  • • Completed portfolio of supervised practice in advanced clinical practice.
  • • Evidence of continued professional development following MSc completion.
  • • Evidence of recent teaching, mentorship, and supervision of staff
  • · Evidence of multi – professional working
Desirable criteria
  • Evidence of clinical work in an autonomous role
  • · Evidence of quality improvement methodology and audit experience
  • · Evidence of involvement in change management
  • · Working towards a full MSc in Advanced clinical practice

Qualifications

Essential criteria
  • • Professional Registration with either NMC, GPhC or HCPC.
  • • Possession of 1st degree in a clinical field e.g., Nursing, physiotherapy at a minimum 2:2 classification
  • • Non-Medical Prescribing (v300) if appropriate to registered profession /role
  • • Leadership/management course
Desirable criteria
  • • Evidence of continuing education
  • • Working towards an MSc in Advanced Practice or recognised equivalent by HEE Centre Advancing Practice
