Job overview

An exciting opportunity has risen for enthusiastic and motivated individuals to join our Reactive Emergency Assessment Community Team (REACT) as a Senior Registered Nurse. The REACT team is a multi-disciplinary integrated community service within ESNEFT incorporating OPAT (Outpatient Parenteral Antibiotics Therapy), OOHs (Out of Hours) and Virtual Ward pathways patients, part time 22.5 hours per week.

Band 5 to Band 6 developmental role would be considered.

Our team delivers excellent patient focused care to patients in the East Suffolk community. Could you be a valued key member of our team? We will offer an individually tailored induction, training and support for the successful candidate.

The role involves travelling throughout East Suffolk delivering acute care in patient’s home as a part of REACT nursing team, both during core hours and out of hours. You will be working in an internal rotation. You will be expected to work 11-11 triage shifts with some early, late and night shifts including weekends and bank holidays as the service requires. During out of hours between 18:00-8:00 the Nurse will be accompanied with a Health Care Assistant.

Pool cars are accessible for staff to use and it is essential that you have a valid driving licence.

Main duties of the job

In this role, during the core hours, teams’ main responsibilities include working in partnership with the OPAT (Outpatient Parental Antibiotic Therapy) to administer intravenous antibiotics via PICC line or cannula for patients receiving antibiotics in community. Core hours will also include assessment and delivery of care to patients in crisis to allow admission prevention. During the out hours main responsibilities include routine and emergency response to wound care, catheter care for all types of catheters, diabetic care, End of Life care, Anticoagulant medication, Pressure Area care, Venepuncture and Sub cut Fluids in order to ensure 24 hours Nursing care cover in community. You would be liaising with District nursing service to ensure continuity of care and another community services where needed.

The role will also include being the shifts team lead, triaging and co-ordinating case load, scheduling visits, carrying out appraisals and clinical supervision for more junior staff. Part of this role is also to support senior management in daily running of the team and promoting change. You should be motivated, flexible and committed to working in a changing environment.

  • Sound evidence of clinical knowledge

  • Provides evidence of ability to make changes in nursing practice
