An exciting opportunity has emerged within South East Essex Community Health Services to be part of the inpatient Intermediate Care / Stroke facility at Rochford Hospital.

At Essex University Partnership NHS Foundation Trust, we are looking for talented and caring Healthcare Assistants to join our inpatient team.
To assist the senior team in providing care and physical rehabilitation to people who require short term period of recovery outside their own homes following a an acute event or deterioration that may or may not have resulted in an acute hospital stay. The aim is to provide person centred rehabilitation to enable patients to reach their maximum potential and live as independently as possible.

Main duties of the job
- Encourage patients to participate in activities of daily living and facilitate their active involvement in therapeutic activities.
- Assist the patient work towards rehabilitation goals to enhance independence and wellbeing.
- Ensure active involvement in manual handling of patients as required – in line with the Trust’s Manual Handling Policy.
- To work within the guidance of the Infection Control Policy and wearing appropriate protective clothing at all times.
- Observe record and report changes in the patient’s health and environment to senior staff.
- To be familiar with the steps to be taken in medical emergencies and act accordingly.
- Complete observations including urinalysis and blood sugar monitoring as required.
- Collect patient specimens as required.
- Observe and record dietary needs and fluid intake as required.
- Assist the senior team with visits case conferences etc. as necessary.
- Work directly with the therapists within the team to support them providing effective assessment and treatment.
- Assist in planning work together with the senior Team.
- Assist in contributing to care planning and risk management plans.

Person specification
Education
Essential criteria
- Good standard of education to GCSE or equivalent
- Completed Care Certificate (or within 6month of commencement)
Skills / Experience
Essential criteria
- Effective oral/written communication and interpersonal skills
- Computer skills and knowledge of Microsoft Office
Desirable criteria
- Experience in rehabilitation with a positive approach to rehabilitative care
- Experience providing care in a Ward Setting
- Previous experience of SystmOne
Personal Qualities
Essential criteria
- Strong commitment to working as part of a team.
