Community Dementia Nurse Practitioner

Mental health services

  • We provide a wide range of treatment and support to young people, adults and older people experiencing mental illness both as inpatients and within the community. This includes treatment in secure and specialised settings.

Community health services

  • Our diverse range of community health services provide support and treatment to both adults and children. We deliver this care in community hospitals, health centres, GP surgeries and in people’s homes.

Learning disabilities services

  • We provide crisis support and inpatient services and our community learning disability teams work in partnership with local councils to provide assessment and support for adults with learning disabilities.

Social care

  • We provide personalised social care support to people with a range of needs, including people with learning disabilities or mental illness, supporting people to live independently.

South East Essex Dementia and Older People's Community Mental Health Services are excited to be able to advertise band 6 registered nursing roles in their successful Dementia Intensive Support Team (DIST). The DIS team has continued to expand as a service, working both in the community and in Southend University Hospital providing a liaison service in the Acute Trust.

The team is based at the Harland Centre, Westcliff-on-Sea, with office space there and at Southend University Hospital. DIST is part of South East Essex Dementia Services providing services across the Castle Point & Rochford and Southend area. The services includes Older People’s CMHT, Community Dementia Nursing (CDN) Team and the Memory Assessment Service (MAS) and comprises of Mental Health Nurses, Associate Practitioners and Allied Health Practitioners.

The DIS Team is an integral part of the service and work jointly with the Acute Trust, Community Health Services, Mental Health, Primary Care, Intermediate Care and other agencies, including social care, Essex ambulance services and third sector originations.

The successful candidate will be able to demonstrate:

An awareness of how dementia affects the individual and their families.

Experience of working with people with dementia in community settings.

A background of providing person centered care and have excellent knowledge and/ or experience of working in dementia services.

Skilled leadership with an ability to grow and develop others at all levels of our division.

The ability to offer and provide advice to support the management of decision-making in relation to complex cases.
