Registered Nurse - Day Assessment Unit

Mid and South Essex NHS Foundation Trust is the only acute hospital provider in the Mid and South Essex Integrated Care System (ICS), which is a system of health and care partners working together for our local population.

Job overview

The Day Assessment Unit (DAU) are looking for a compassionate and caring Registered Nurse to join their bright and friendly team.

We believe team work is at the centre of our success.  We are a very supportive team and believe that if we work together we can provide excellent patient care. This unit is part of the Elderly Care and Frailty Team. 

The Day Assessment Unit is an admission avoidance unit where we aim to discharge patients back to the community under the care of our community colleagues.  We run many outpatient clinics too alongside our bedded assessment area.

Our aim is to provide a supportive environment fostering person-centred care to enable patients to reach their optimum level of health and independence. We treat frail patients, and so excellent levels of compassion and patience are required and experience of caring for people living with dementia would be preferable.

If you are have great communications skills and an empathetic, caring approach to your nursing then DAU could be the team to join.

Main duties of the job

The Unit:

DAU  is a dynamic and friendly team that believes team working is at the centre of our success.  We are a very supportive team and believe that if we work together we can provide excellent patient care.

To complete full Nursing Assessments and investigations within the concept of Comprehensive Geriatric Assessments, implement treatment/care plans and arrange safe discharge or admit and handover the care to an inpatient ward.

To ensure that an individual programme of care is planned, implemented and evaluated for each patient within own sphere of responsibility.

To work as part of the multidisciplinary team ensuring the delivery of a high standard of care to the patients.

To act as team leader including taking charge of the ward in the absence of the Ward Manager/Deputy Ward Manager.

To participate in all relevant aspects of the Governance process.

You will adhere to the NMC code of Professional Conduct at all times and ensure compliance with hospital policies and procedures.

To act as a positive role model at all times.
