Discharge Facilitator

Mid and South Essex NHS Foundation Trust is the only acute hospital provider in the Mid and South Essex Integrated Care System (ICS), which is a system of health and care partners working together for our local population.
Within Mid and South Essex there are 149 GP practices, operating from over 200 sites, forming 27 Primary Care Networks and one Ambulance Trust. There are also three main community and mental health service providers who work together through a community collaborative.
We collaborate with other valuable partners, including three Healthwatch organisations, nine voluntary and community sector organisations and three top tier local authorities, with seven district, borough and city councils, as well as our local universities.
Job overview
We have an exciting opportunity for a Discharge Support Worker with administrative experience to join our team on a busy 35-bedded short stay ward.
This ward experiences a high turnover of patients, providing a dynamic and fast-paced working environment. You will have the opportunity to work closely with the ward’s Integrated Discharge Team (IDT) Facilitator to support safe and timely discharges.
The ward is short stay unit with high turnover of patients, opportunity to work alongside IDT facilitator for the ward.
Good communication skills as this job requires excellent communication. with patient families.
A working knowledge of TeleTracking is desirable.

Main duties of the job
- Provide support to nursing staff and the medical team.
- Complete referrals to Social Services and ensure appropriate follow-up.
- Coordinate with families regarding patient discharges and liaise with care homes as needed.
- Manage and submit pharmacy requests.
- Communicate with the Patient Flow Team to support efficient ward operations and follow up on pending investigations.

Qualifications
Essential criteria
- Good standard of education including maths and English
Desirable criteria
- Specialist course relevant to clinical area.
- Experience in discharge planning
- Evidence of working towards Foundation degree in health related subject
Communication

- Excellent written and verbal communication skills, report writing.
Knowledge and Experience
Essential criteria
- Able to plan, implement and evaluate patient care.
