Discharge Facilitator

Job overview

We have an exciting opportunity for a Discharge Support Worker with administrative experience to join our team on a busy 35-bedded short stay ward.

This ward experiences a high turnover of patients, providing a dynamic and fast-paced working environment. You will have the opportunity to work closely with the ward’s Integrated Discharge Team (IDT) Facilitator to support safe and timely discharges.

The ward is short stay unit with high turnover of patients, opportunity to work alongside IDT facilitator for the ward. 

Good communication skills as this job requires excellent communication. with patient families.

A working knowledge of TeleTracking is desirable.

Main duties of the job

  • Provide support to nursing staff and the medical team.
  • Complete referrals to Social Services and ensure appropriate follow-up.
  • Coordinate with families regarding patient discharges and liaise with care homes as needed.
  • Manage and submit pharmacy requests.
  • Communicate with the Patient Flow Team to support efficient ward operations and follow up on pending investigations.

Qualifications

Essential criteria
  • Good standard of education including maths and English
Desirable criteria
  • Specialist course relevant to clinical area.
  • Experience in discharge planning
  • Evidence of working towards Foundation degree in health related subject

Communication

Essential criteria
  • Excellent written and verbal communication skills, report writing.

Knowledge and Experience

  • Able to plan, implement and evaluate patient care.
