Job overview

Gloucestershire Hospitals NHS Foundation Trust is seeking to appoint an Advanced Clinical Fellow in Gastroenterology to join our well-established and highly regarded Gastroenterology and Endoscopy service within the Medicine Division.

This is a full-time (40 hours) post aimed at senior Gastroenterology trainees (ST6–8) or post-CCT doctors (or equivalent), offering a structured opportunity to further develop advanced endoscopic skills alongside specialist gastroenterology experience. The post combines 50% service delivery with 50% training, development and research, tailored to the interests and experience of the successful candidate.

The role is based primarily at Cheltenham General Hospital, with cross-county working at Gloucestershire Royal Hospital and community sites. The Trust is home to the Regional Endoscopy Training Centre, providing a strong training, education and research environment, including exposure to advanced lower GI therapies, complex polypectomy, and specialist clinics such as IBD and hepatology

Main duties of the job

The postholder will deliver high-quality clinical care within Gastroenterology and Endoscopy services, contributing to both elective and specialist activity while developing advanced endoscopic and training skills.

Key duties include:

  • Independent performance of upper and lower GI endoscopy, including colonoscopy with level 2 polypectomy (minimum), contributing to service lists

  • Participation in advanced endoscopy training opportunities, including complex EMR, lower GI ESD and colonic dilatation

  • Delivery of outpatient gastroenterology clinics, including opportunities in specialist areas such as IBD, hepatology and nutrition (depending on experience and interests)

  • Active involvement in endoscopy education and training, including opportunities to observe and contribute to regional training courses

  • Participation in audit, quality improvement and research activity, including opportunities in advanced endoscopy and AI-related research

  • Working collaboratively within multidisciplinary teams across gastroenterology, surgery, radiology, histopathology, specialist nursing and allied health professionals

  • Maintaining professional standards in line with GMC Good Medical Practice and Trust clinical governance requirements 

There is no General Internal Medicine commitment and no on-call requirement associated with this post.

Detailed job description and main responsibilities

The Advanced Clinical Fellow will work under the supervision of Consultant Gastroenterologists and will be expected to practice with an appropriate level of autonomy, consistent with their experience and competencies.

Clinical responsibilities

  • Provide high-quality, patient-centred gastroenterology and endoscopy care

  • Undertake independent diagnostic and therapeutic endoscopy, including colonoscopy and gastroscopy

  • Apply sound clinical judgement in the assessment and management of gastroenterology patients

  • Contribute to specialist services including IBD, hepatology, nutrition and bowel cancer screening, as appropriate

  • Participate in multidisciplinary meetings, including IBD, hepatology, radiology and histology MDTs.

Training, education and development

  • Develop advanced endoscopic skills through supervised and independent practice

  • Contribute to the education and supervision of junior doctors, trainees and other healthcare professionals

  • Engage with regional and local endoscopy training activity, with opportunities to develop faculty skills

  • Maintain a professional training portfolio and participate in appraisal and development planning.

Research, audit and quality improvement

  • Participate in clinical audit, quality improvement and patient safety initiatives

  • Engage in research activity where appropriate, including opportunities in endoscopy training, advanced therapy and AI

  • Support the Trust’s clinical governance framework and continuous improvement culture.

Professional and organisational responsibilities

  • Work collaboratively within multidisciplinary and multi-professional teams

  • Comply with Trust policies, clinical governance standards and information governance requirements

  • Maintain GMC registration, licence to practise and mandatory training

  • Uphold the Trust’s commitment to safeguarding, equality, diversity and inclusion.

Essential criteria
  • Demonstrates commitment to delivering high-quality, safe and patient-centred care in Gastroenterology and Endoscopy
  • Demonstrates probity, professionalism and adherence to GMC Good Medical Practice
  • Evidence of participation in clinical audit, quality improvement or patient safety initiatives
Desirable criteria
  • Evidence of completed audit cycles and contribution to service improvement
  • Demonstrates understanding of clinical governance and risk management processes

Knowledge

Essential criteria
  • MBBS (or equivalent) with full MRCP (UK) or equivalent
  • Full GMC registration with licence to practise at time of appointment
  • Sound knowledge of core Gastroenterology and Hepatology practice
  • Knowledge of advanced endoscopic techniques and pathways
Desirable criteria
  • Knowledge of research methodology and critical appraisal
  • Awareness of NHS structures, resource management and service delivery

Experience

Essential criteria
  • ST6+ experience in Gastroenterology and Endoscopy (or equivalent)
  • Independent practice in colonoscopy (minimum level 2 polypectomy)
  • Evidence of competence to work without direct supervision
  • Current ALS certification (or equivalent)
Desirable criteria
  • Experience in teaching or training junior doctors or allied professionals
  • Experience in advanced endoscopic therapy (e.g. complex EMR, ESD, dilatation)
  • Evidence of involvement in research or publications
