The postholder will be an integral part of the radiotherapy late effects team, working under the supervision of the late effects radiographers.  The main purpose of the role is to support people to navigate the complex health and social care system following completion of cancer treatment, specifically radiotherapy.  

The post holder will coordinate care by delivering emotional and practical support to patients who have completed radiotherapy treatment for a cancer diagnosis, and are living with debilitating life-changing late effects. They will work alongside the radiographer team to deliver a person-centred service which identifies and addresses patient concerns. The post-holder will work with a wide range of professionals and services in the hospital setting as well as building links with primary/tertiary and community services.  The post holder will lead in the delivery of holistic needs assessments, patient reported outcome measures and other specific needs assessments, referring to prior care planning and delivery ensuring care delivery is joined up and cohesive.

- Provide general information and support about radiotherapy late effects services

- Engage in daily and direct (e.g., face to face, virtual and telephone) communication with patients.

- Supporting patients living with the consequences of radiotherapy treatment following a cancer diagnosis

- Establish a process of referral and screening of patients. Undertaking patient assessments and formulating individual care plans to best suit patient needs and preferences

- Undertake administrative tasks  to ensure smooth pathways of referrals and patient care including monitoring of database, referrals and clinics

- Support people to access appropriate information and support, by sign-posting to a range of support services and take an approach which helps people to self-manage where appropriate

The Radiotherapy department at Cheltenham General Hospital treats over 3300 cancer patients a year. The department operates as one large team. Despite being a busy department, the caring atmosphere is evident and the patient care provides real job satisfaction for the staff. 

The Late Effects Team provides care and support for patients months and years after their radiotherapy has finished, ensuring that they live well beyond cancer. Whilst the Late Effects Team is small, you will be an integral part of the larger radiotherapy team and play a significant role in ensuring our patients receive the very best quality of care.
