Job overview

We are seeking an enthusiastic Band 6 health care professional who will provide specialist care co-ordination within our Specialist Community Forensic Team (SCFT).

You will have experience of working with people with severe and enduring mental health problems including forensic experience and have a qualification equivalent to degree level in a clinically relevant discipline.

As an ideal candidate for this post you would be a highly competent mental health practitioner looking for a new challenge and seeking an opportunity to make a real difference to people’s lives. We are seeking a motivated individual, if you have an interest in forensic care and have a passion in providing specialist, diverse and collaborative approaches then this vacancy may be for you.

Main duties of the job

  • To provide clinical support, consultation and advice, working freely within the 4 Tier model.
  • To manage a case load of patients with complex needs including forensic and risk management. With support from other team members provide a robust service and management to our patients.
  • To ensure the delivery of expert assessments and interventions and ensure care is delivered effectively and to a high standard. Ensuring that everybody involved the case are involved including other professionals, outside agencies, carers and family.
  • Ensure the delivery of consistent care, professional boundaries are maintained and based around evidence based in accordance with best practice.
  • To ensure we are able to clearly identify risks and any safeguarding concerns and escalate accordingly.

Qualifications

Essential criteria
  • Registered Professional at degree level or equivalent (MH Nursing/ OT/Allied Health Professional)
  • Evidence of experience
Desirable criteria
  • Experience working in Forensic care

Knowledge

Essential criteria
  • Knowledge and understanding of the Mental Health Act & legal aspects of Mental Health care. Including Forensic sections and (MOJ) Ministry of justice.
  • Demonstrable skills in risk assessment and management.
  • Ability to undertake vital assessments in relation to profession.
  • Ability to work on social factors affecting mental distress e.g. accommodation, substance misuse, domestic violence, welfare rights
Desirable criteria
  • Up to date clinical knowledge including risk assessment, dual diagnosis and health and social care needs

Personal skills and attributes

Essential criteria
  • Approachable and flexible Good communication skills (written and verbal)
  • Ability to undertake all statutory and mandatory
  • Ability to work effectively as a team member
  • Ability to assimilate new concepts and approaches
Desirable criteria
