We are looking for caring, motivated and enthusiastic individuals to join our new countywide Home First service as Home First Assistants. Working directly with people in their own homes following hospital discharge, you will play a key role in supporting recovery, promoting independence and helping people achieve what matters most to them.

As part of a supportive multidisciplinary team, you will provide personalised care and reablement, undertake assessments, review progress and support individuals to regain everyday skills and confidence. You will work closely with therapists, nurses, social care colleagues, families and carers to ensure people receive the right support at the right time.

There is a wide range of shift patterns available between the hours of 7am-10pm on a four-day-on, four-day-off shift pattern. These roles are available over multiple sites in the East Gloucestershire team, which includes Cheltenham, Stroud and the Cotswolds. 

Main duties of the job

As a Home First Assistant, you will support adults in their own homes following hospital discharge, helping them to regain independence, confidence and daily living skills. Working both independently and as part of the wider Home First multidisciplinary team, you will deliver person-centred support and reablement interventions tailored to individual goals and needs.

You will undertake holistic assessments and risk assessments, support people with daily living activities, and provide practical, personal and, where required, intimate care. You will assist individuals to achieve agreed outcomes, monitor progress and identify changes in need, escalating concerns appropriately to senior team members.

The role includes supporting people to use equipment safely, undertaking basic clinical observations, promoting wellbeing and independence, encouraging engagement with community resources and digital technology, and helping people maintain or rebuild links within their local community.

You will maintain accurate clinical records, contribute to reviews and multidisciplinary discussions, and work collaboratively with therapists, nurses, social care colleagues, families and carers to ensure safe and effective care delivery.

The successful candidate will be comfortable working independently within the community, able to manage changing priorities and committed to delivering high-quality, compassionate care that enables people to remain safely in their own homes wherever possible.

This high-level overview shows we are in a healthy position, with higher scores than average for comparable organisations, alongside a great response rate, indicating good staff engagement. However, we also know we have plenty of room for improvement in many areas. To that end, we continue to prioritise and invest in our commitment to genuinely becoming a Great Place to Work with consistent top-quartile performance in the annual staff survey and Pulse surveys.
