The Countywide directorate covers a comprehensive range of services offering care and support to improve peoples’  physical health and wellbeing, including Long Term Conditions, Equipment and Therapies, Sexual Health, and Dentistry

The Community Neurology Service (CNS)  is a multi-disciplinary team providing specialist rehabilitation and management of long-term neurological conditions.  The service offers practical support, advice and therapy to service users, carers and relatives and other healthcare professionals.  The team includes specialist neurology physiotherapists, occupational therapists, speech and language therapists, clinical psychologists, therapy support workers and administrators who are based at Edward Jenner Court.  The CNS delivers its service countywide via home visits, outpatient clinics and virtual or telephone appointments. 

 This post will be responsible for service delivery across the County, including Cotswolds locality (North and South Cotswolds)

Main duties of the job

We are looking for a Clinical Specialist Physiotherapist with extensive postgraduate clinical experience to join GHC NHS Foundation Trust’s  Community Neurology Service (CNS) working predominantly within Cotswold locality.

The Clinical Specialist Physiotherapist  in Stroke/Neurological Rehabilitation services will provide expertise in the assessment, diagnosis, and treatment of patients with neuro and/or complex presentation.  

You will be an integral member of the multidisciplinary team in Gloucestershire, acting as a resource to other medical, nursing, therapy and social services staff and voluntary agencies and as an interface link  with acute colleagues.  You will work alongside Band 5, 6 and 7 Physiotherapy colleagues providing clinical expertise, teaching and service development in the field of neurological rehabilitation.

Detailed job description and main responsibilities

Undertake all aspects of clinical duties as an autonomous practitioner and work as a highly skilled specialist clinician in the field of community rehabilitation and as a lead for an identified care pathway.

Undertake the comprehensive assessment and accurate diagnosis of patients including those with a complex presentation using investigative, analytical and advanced clinical reasoning skills frequently, often with no access to other colleagues.

Essential criteria
  • Significant post graduate neurology education (level 7) and neurology specific experience at senior level
  • Evidence of leading change management and service development
  • Evidence of planning, organising and evaluating formal teaching in neurological field
  • Experience of evaluating clinical practice using current research, guidelines, audit/outcomes
Desirable criteria
  • Clinical Educator training and experience supporting students

  • Specialist knowledge in Neurology including MND, SCI and Stroke

  • Complex MDT treatment and discharge planning
