Job overview
This is an opportunity to join the Diabetes Service in Gloucestershire to play a pivotal role in shaping how Diabetes and Obesity care is delivered. Gloucestershire’s Integrated Care System (ICS) is on an exciting journey to deliver more place-based, joined-up care for our population, as per the “NHS Ten Year Plan”. We’re proud to be a progressive, supportive health system with strong partnerships across acute, community, and primary care. The aim is for integrated diabetes care across all settings and the successful applicant will play an important role in the delivery of this model of working.
In Gloucestershire, we are working as a healthcare community to improve the identification and management of diabetes and obesity, tackle health inequalities, and support the wellbeing of the local population. By 2029, we are committed to closing the gap in morbidity and mortality between the most and least deprived areas, while also reducing demand on acute services through strengthened, community-based care.
This is the development of an established role within our system, working as part of the acute trust team based in the community. This is a fantastic opportunity for a motivated clinician focused on treating type 2 diabetes and obesity to help shape and further develop our model for diabetes services in the county. Main duties of the job
Integral to the responsibilities of the post are the following requirements:
- Lead the Community and Complex Type 2 Diabetes Service
- Providing training, education, and support to professionals across the system
- Contribute to system wide pathway review and development
- Conduct primarily community-based outpatient clinics for adults with complex type 2 diabetes, and a focus on developing a specific under 40 type 2 DM clinic using weight management experience to address the underlying issues
- Provide care for patients with type 1 diabetes who are unable to attend appointments in the hospital setting
- Provide structured clinical governance to the Community Diabetes Specialist Team
- Collaborate with the multidisciplinary team (MDT) including Diabetes Specialist Nurses, Dietitians, a Nutritionist, Psychologist, Health Educators, Primary care Champion, and Podiatrists.
- Work with the Acute Diabetes and Endocrinology team at Gloucestershire Hospitals NHS Foundation Trust.
- A day per week with the Gloucestershire Tier 4 Specialised Weight Management Service for those patients without diabetes living with obesity.
- Engage with Integrated Locality Partnerships and Primary Care Networks to develop the service.
- Continue and develop MDT and mentored diabetes clinics focusing on education within Primary Care Network.
- Involvement in the education programs for diabetes.
- Actively participate in audit and research

Detailed job description and main responsibilities
The purpose of this post is to provide a substantive Consultant for the Community and Complex Type 2 Diabetes service. As a senior employee of the Trust the post-holder will work in close co-operation with, and support other clinical, medical professional and managerial colleagues in providing high quality healthcare to the Trust’s patients.
This is a replacement post for a consultant diabetologist with an interest in complex type 2 diabetes and community diabetes alongside addressing metabolic risk factors including weight management. It is an opportunity to continue to develop the service, so a track record of leadership and service development is desirable. The vision is that the integrated services will bring together primary, community in secondary care adult diabetes services to create a service without boundaries for Gloucestershire. Our clinical outcomes for people with diabetes are good but we want to make them even better. We aspire to excel and achieve national recognition for service provision.

Qualifications and Training
Essential criteria
- Full General Medical Council registration
- On Specialist Register for General (Internal) Medicine and Generic Medicine via CCT or CESR-CP (Combined Programme) (Proposed CCT/CESR-CP date must be within 6 months of interview

- Diplomas or postgraduate thesis relevant to the specialty

- Ability to appraise research critically and apply research outcomes to clinical and surgical problems

Clinical/Experience
Essential criteria
- Clinical training & experience equivalent to that required to obtain UK CCT in Diabetes, Endocrinology & General Medicine
- Ability to take full and independent responsibility for clinical care of patients
Desirable criteria
- An interest in obesity (weight management)
- Experience in community-based care provision
