Job overview

Clinical Nurse Specialist Stoma Care, Band 6 (13.12 hours a week)

An exciting opportunity has arisen for a motivated Stoma Nurse to deliver evidence based quality care to around 2,500 stoma patients in Gloucestershire. They will act as a resource for registered nurses, students and HCA’s on the wards requiring specialist advise to support and participate in stoma education and teaching.

They will teach patients with newly formed stomas and review long term stoma patients; they will teach parents to care for their children with newly formed stomas.

The role will involve stoma care both in the Acute hospital and the community. The Stoma Care team also operate a telephone help line.

Main duties of the job

The successful candidate will act as a resource and support for nursing teams in care delivery.

They will tech stoma care to patients to promote independence and enhance quality of life.

They will ensure cost effective use of stoma prescription items and evidence the same to the ICB via GP surgeries and Appliance order Line.

They will answer queries raised by the telephone help line and will run community and acute clinics either “face to face” or virtually.

They will regularly review stoma product usage.

The Stoma Care Team in Gloucestershire provide education and teaching for 2,500 patients who have newly formed or established stomas, the team work in both Cheltenham and Gloucestershire hospitals and in the community. 

The Stoma Care team have full access to all the fully accredited masterclasses run in the UK.

The Stoma care team currently operate a 6 day a week service and some Bank holiday working may be expected. 

Detailed job description and main responsibilities

- Responsible for assisting with the daily delivery of stoma care, pre and post operatively and for operating community nurse led clinics and performing home visits.

- Responsible for delivery of Stoma Care education programmes both for the development of staff on the wards and also in Care homes.

- Be able to work alongside nursing staff, role modelling high standards of practice whilst assessing, delivering and evaluating patient care. Highlighting any areas of development for team members as required.

- Responsible for the administration of clinical procedures/treatments within Stoma Care, acting as an expert resource for the surgical wards and the Stoma team.

- To effectively input and retrieve patient information from computer based systems, specifically Trak, Mime and EPR.

- Maintain accurate and contemporaneous patient records.

- To administer medication and treatment within the Procedure for the Ordering, Prescribing and Administration of Medication Guidelines and contribute to the development of speciality PGD’s.

- To carry out patient prescription reviews as required in GP practices and Appliance order Line.

- To deliver safe care within a challenging and changing clinical environment, using equipment and resources as safely and as appropriate.

 - Participate in audits and undertake local research relevant to Stoma care.

- To adhere to all Trust policies and procedures and professional code of conduct.

- Provide support for local patient associations.

Qualifications, Knowledge and Experience

Essential criteria
  • NMC registration
  • Appropriate post registration experience with experience in stoma care
  • Colorectal or urological experience with experience in Stoma care
  • Completion of extra stoma training eg foundation course
  • Excellent organisational and interpersonal skills
  • Confident and able to work independently
  • Demonstrate ability to use clinical reasoning skills
  • Have knowledge of research and audit
  • Act as a role model and lead by example
  • Be able to demonstrate a specialist knowledge of stoma care
Desirable criteria
  • Will be required to travel to community hospitals and patient homes.
  • Have or be working towards a level 6 qualification
