We are seeking an experienced and highly motivated Discharge & Flow Programme Lead to provide system-wide leadership for one of the most critical priorities across our Integrated Care System.

We are looking for a senior leader with significant experience in discharge & flow,  and a strong track record of delivering improvement across multiple organisations. You will be confident using data and performance metrics to drive action, and comfortable operating in a complex, fast-paced environment where collaboration and outcomes matter.

This is an exciting opportunity to shape and deliver system-wide improvement that makes a real difference to patient experience, flow and outcomes across Hampshire & Isle of Wight or a similarly complex system.

Main duties of the job

This senior role will lead the delivery of the ICS Discharge and Flow Programme, driving sustained reductions in Non-Criteria To Reside (nCTR) and Delayed Readiness for Discharge (DRD) across all acute sites. The postholder will play a pivotal role in embedding a Home First / Discharge to Assess (D2A) approach and delivering a single, fully integrated Transfer of Care Hub (TOCH) model spanning health, social care and the voluntary sector.

Working in close partnership with acute trusts, community providers, local authorities and system leaders, you will provide pace, grip and expert leadership across organisational boundaries. This is a high-profile, delivery-focused role requiring strong influencing skills, operational credibility and a proven ability to deliver complex change at scale.

Detailed job description and main responsibilities

Lead the end-to-end design and delivery of the system Discharge & Flow Programme, aligned to ICS priorities and national policy.

Act as the system lead for reducing nCTR and DRD, ensuring clear trajectories, grip and delivery across all partners.

Support the overall initiation and delivery of major cross-organisational change projects, as required.

Translate national discharge policy and guidance into practical, deliverable system-wide models and standards.

Ensure all projects deliver financial and operational improvements as well as improvements in patient experience, clinical quality and staff engagement.

Champion and lead transformation and change across the portfolio, working with wider ICS teams and Clinical leaders to bring forward evidence-based innovation and solutions which deliver against the outcomes.

Segment and manage multiple stakeholder groups appropriately in a complex multi- disciplinary environment. Work closely with the Communication and Engagement teams to engage with all the appropriate stakeholder groups, capturing outputs to inform business cases and further action. Ensure all stakeholder interests are identified and addressed, including clinicians, patients/carers, regulators, system partners and the public.

Manage communication between stakeholders relating to integrated Discharge delivery; use expert negotiation skills to present complex, potentially controversial information to large groups and use negotiation skills to resolve issues that may arise during project implementation.

Qualifications and/ or experience

Essential criteria
  • Educated to master’s level or equivalent level of experience of working at a senior level in specialist area. Evidence of post qualifying and continuing professional development.
  • Extensive knowledge of specialist areas, acquired through post graduate diploma or equivalent experience or training plus further specialist knowledge or experience to master’s level equivalent.
  • Evidence of post qualifying and continuing professional development
Desirable criteria
  • Qualification in Project Management – APM, Prince or similar

Skills and abilities

Essential criteria
  • Significant experience of managing or supporting large change programmes within the NHS, and specifically within the UEC and Discharge environments
  • Significant experience working across organisational boundaries and with conflicting incentives

Knowledge

Essential criteria
  • Demonstrable success & knowledge in delivering large scale operational, financial and cultural change and performance
  • Significant experience of leading, managing, developing and motivating teams of skilled professionals.
  • Highly experienced in leading major change initiatives in a challenging organisational environment
  • Ability to critically analyse extremely complex performance, financial and clinical datasets.
  • Able to effectively manage performance by holding themselves and others to account for service outcomes
  • Extensive understanding of Discharge processes within Acute, community provider and Local Authority settings

Communication and interpersonal skills

Essential criteria
  • Experienced in coaching executives and senior teams
  • Highly experienced in leading, managing, developing and motivating teams of skilled professionals
  • Understands the underlying social, political and historical factors shaping local realities of health services and uses understanding of politics of health and social care to get things done
  • Provide and receive highly complex, highly sensitive and highly contentious information, negotiating with very senior stakeholders across many organisations on difficult and controversial issues, and presenting highly complex and highly sensitive information to large and influential groups, often in an highly emotive atmosphere

  • Able to demonstrate sound political judgement and astuteness in understanding the contexts for change
  • Ability to analyse highly complex problems and to develop practical and workable solutions using knowledge and evidence
  • Able to make sound decisions using all available evidence, and evaluate their impact
  • Excellent problem solving skills and proven successes in the ability to respond to sudden unexpected demands.

IT and digital literacy

Essential criteria
  • Extensive experience in using all MS Office applications
  • Highly skilled in using data sets, SPC charts, and other digital tools to drive decisions and communicate opportunities to stakeholders
Desirable criteria
  • Experience using Project Management Software
