Patient Flow Facilitator

Job overview
An opportunity has risen for a Patient Flow Facilitator to support with planning patient discharges on E3 ward.
The suitable candidate will have excellent communication skills and will be expected to arrange transport, liaise with patients and their families, and members of our multi-disciplinary team to help plan safe discharges for our patients.
Main duties of the job
To co-ordinate multiple aspects of care to ensure progression of rehabilitation, continuity of care and timely discharge planning for patients.
To co-ordinate services associated with discharge from the ward, e.g. pharmacy, transport, referrals to external agencies, to ensure timely discharge of patients
Facilitate multi-disciplinary meetings and board rounds with a focus on continuity of care, goal setting and safe and timely discharge
Co-ordinate patient flow, ensuring timely discharges from the ward.
Organise and facilitate planning meetings to organise safe, timely discharges with patients, relatives and the multi-disciplinary team
Build close relationships with the Complex Discharge Team and Respiratory Department
Liaise with patients and families to manage expectations of the discharge process and early identification of issues impacting discharge

Training and Qualifications
Essential criteria
- Literacy level 2 or equivalent (GCSE A-C)
Desirable criteria
- NVQ level 3 or above in Health and Social Care or working towards an award.
Experience and knowledge

- Good reasoning, analysing and decision making skills using a variety of resources
Desirable criteria
- Experience of working within the NHS
