A rare opportunity has arisen for a Community Specialist Practitioner within our friendly Community Integrated Care Team in Lower Lea Valley. 

Main duties of the job

The Specialist Practitioner will work within the Integrated team of Community Nurses, Therapists, CPNs and Social Workers. You will have responsibility for ensuring all health and social care needs are met to improve health outcomes, functional status and quality of life. This will lead to increased patient choice and prevent unnecessary admissions to hospital and reduce the stay of necessary hospital admissions.

Detailed job description and main responsibilities

The post-holder will be an excellent communicator and work well both individually and within a team. You will be patient focused and have the ability to undertake clinical assessments to arrive at a differential diagnosis, start, plan and evaluate care. You will also have organisational and time management skills and be able to demonstrate leadership skills across the inter-professional team.  An extensive experience of working with older people is required and evidence of continuing professional knowledge and development.

A full valid driving licence and access to a car to use regularly for business purposes is essential (unless you have a disability as defined by the Equality Act 2010).

In addition to the basic salary, this post attracts a High Cost Area Supplement of 5% of basic salary subject to a maximum payment of £1,845 per annum (pro rata where applicable).

Qualifications and Training

Essential criteria
  • RN First Level Nurse/AHP.
  • Community Nursing Qualification or equivalent post registration advanced AHP qualification.
  • Specialist Practitioner or equivalent AHP.
  • Independent Nurse Prescriber – mandatory to pass and practice.
  • Demonstrates a high level of relevant clinical experience.

Experience and Knowledge

Essential criteria
  • Extensive experience working with older people.
  • Continuing professional knowledge and development.
  • Experience of teaching and mentorship.
  • Change management and leadership skills.

Skills and Abilities

Essential criteria
  • Evidence of ability to communicate both verbally and in writing at all levels in multi-disciplinary forum and with users and carers.
  • Demonstrate competence in ‘top to toe’ assessment
  • Demonstrate competence in safe medicines prescribing
  • Demonstrate competence in phlebotomy
  • Demonstrate competence in administration of intravenous therapy
