Job overview
We are recruiting for an Administrator - Personalised Care Follow Up (PCFU) who shares our vision to be trusted to provide consistently outstanding care and exemplary service to our patients.
The cancer support worker for Personalised Care Follow Up (PCFU) will form an integral part of the multidisciplinary team, alongside the CNS caring for the physical, social, and psychological needs of the patients who have completed their treatment for cancer.
You will assist the MDT in coordinating all appointments, signposting patients as necessary to the relevant team member to support the patient through the self-managed part of their pathway.

Main duties of the job
Act as point of contact for queries relating to scheduling of patient appointments and co-ordination of patient follow up pathways. The patients maybe challenging including verbal aggression at times due to the anxiety of their cancer follow up.
Respond to patient and other Healthcare professional’s communication acting as a triage for patients and ensuring they are signposted to the appropriate member of the team.
Manage the database for all patients on the PCFU, liaising with the MDT coordinator to ensure patients are reviewed as necessary at the weekly MDT.
Be aware of the physical, emotional and psychological needs of the patient. Able to observe, listen and report any changes to the patient’s condition to a member of the MDT so necessary guidance can be given.
Identify to the clinical team, patients who require a Holistic Needs Assessment (HNA) and ensure one is completed prior to any appointments should they be needed.
Support patients who need technical support completing the electronic HNA or remote surveillance software.
Identify patients who are due routine follow up tests and ensure they are aware of their upcoming appointments.

Qualifications / Training
Essential criteria
- NVQ level 4 in health or social care or equivalent experience
Desirable criteria
- Evidence of further study, courses related to healthcare or cancer care.
Previous Experience
Essential criteria
- Dealing with challenging situations

- NHS Experience in administration
Desirable criteria
- Working as part of an NHS Multidisciplinary Team
- Working with Cancer patients
- Experience of using Hospital IT systems

You must have appropriate UK professional registration.
The postholder will have access to vulnerable people in the course of their normal duties and as such this post is subject to the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (Amendment) (England and Wales) Order 2020 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service to check for any previous criminal convictions.
