Job overview

We have a rare vacancy in our team for a qualified and experienced community nurse, this is due to internal movement within the team.

You will be part of a friendly and supportive integrated team based in the integrated care coordination hub.

We are looking to recruit a compassionate, enthusiastic and experienced nurse to provide senior triage nursing support to our team of multidisciplinary professionals.

Whilst the role is mainly required to provide advanced complex nursing assessments to enable triage of patients into the most appropriate pathway there will be times when you are required to assist with observation monitoring of patients under the virtual ward. To enable this flexibility of working suitable training and support will be provided.

Main duties of the job

·  Provide a leadership link between the hub and the Integrated Community Teams (ICTs) to ensure effective communication, resolution of issues, and improve the pathways between the staff and teams.

·  Act as senior triage clinician of the day for ICT referrals received, overseeing complex referrals.

·     Use complex, advanced skills and expert knowledge to identify changes and/ or deterioration in condition and act on these appropriately.

·   To be responsible for the clinical supervision for the Nursing Triage team, including appraisals.

Detailed job description and main responsibilities

        Provide senior triage nursing support to our team of multidisciplinary professionals providing clinical health and social care in admission prevention / avoidance service.

          Maintain standards of care and to deploy, co-ordinate and supervise the nursing team and learners as required.

        Be responsible for the assessment, planning, implementation and evaluation of individualised nursing care to patients living in the community.

          Be highly experienced in management of a wide range of community pathways and provide specialist advice for patient care. They will provide advanced complex nursing assessments which may include on occasion interpreting blood results.

·       The post holder will, assist in triage of complex patients within all the community pathways which may include H at H pathways, having clinician to clinician discussions to facilitate effective referral into the most appropriate service to meet the needs of the patient.

·       The post holder will support patients virtually and when required face to face.

  • Relevant professional degree or diploma
  • Registration with appropriate professional organisation i.e. NMC
  • Professional knowledge supplemented by further training, short courses, experience
