Team Lead - Hospital at Home Service

NHS AfC: Band 7

Job overview

An exciting opportunity has arisen for an experienced and motivated Registered Nurse or HCPC Registered Paramedic to join Hertfordshire Community NHS Trust as a Band 7 Team Leader within our Hospital at Home Service.

Hospital at Home provides safe, responsive and high-quality care to patients in their usual place of residence. The service supports the prevention of avoidable hospital admissions and facilitates earlier discharge by providing assessment, treatment, monitoring and ongoing clinical review for patients with acute illness, frailty, long-term conditions, palliative care needs and other complex presentations.

The post holder will provide visible clinical and operational leadership across the Hospital at Home, Remote Monitoring and Home Health Monitoring pathways. Working closely with medical colleagues, nursing and paramedic staff, therapists, pharmacists, Integrated Neighbourhood Teams, GP practices and other system partners, the Team Leader will help ensure that patients receive coordinated, evidence-based and person-centred care.

This is a varied role combining advanced clinical practice with operational coordination, staff leadership, workforce management, clinical governance and service development.

The role is based at Abel Smith House in Stevenage and requires flexible working across a seven-day service and weekends

*Proposed interview date: 17th November 2026*

Main duties of the job

The Team Leader will provide clinical and operational leadership for the Hospital at Home Service, supporting safe patient flow, effective use of service capacity and the day-to-day deployment of the workforce.

The post holder will:

  • Triage complex referrals and undertake clinician-to-clinician discussions to support safe admission to, movement within and discharge from Hospital at Home pathways.
  • Undertake and support advanced, holistic patient assessment, using clinical observations, available investigation results and professional judgement to identify deterioration and help formulate appropriate management plans.
  • Coordinate the Hospital at Home, Remote Monitoring and Home Health Monitoring caseloads, balancing patient acuity, service capacity, staffing and competing priorities.
  • Provide clear clinical and operational direction to staff and escalate risks or concerns appropriately.
  • Participate in multidisciplinary meetings, case discussions and virtual or face-to-face ward rounds.
  • Provide visible day-to-day leadership and line management to an identified group of staff.
  • Support staff supervision, appraisal, wellbeing, competency assessment, performance management and professional development.
  • Lead or contribute to clinical audit, quality improvement, service evaluation, incident learning and clinical governance activity.
