This is a new and unique vacancy!  

The role will be responsible for the MH assessment, planning, implementation, evaluation of care for service users on 136.  You will work in collaboration with the Mental Health Liaison Team based at the Lister Hospital in Stevenage.

You will manage the 136 resources and clinical environment where the 136 patient is during shift and provide reports to the senior management team on the 136 activity in the Emergency dept.  When there are no 136 patients in the ED, you will have the opportunity to revolve into the MHLT assessor role.

Main duties of the job

  • Will function as a 136 Flow coordinator nurse within acute trust setting
  • Providing care and oversight of individuals or group of service users as allocated, taking responsibility for formulation of appropriate care plans in line with 136 policy
  • Will coordinate all co-workers assigned to his/her service user group and will ensure they are all fully conversant with all aspects regarding service users care and treatment
  • Will identify the individual care needs of allocated services users plan, implement, and evaluate all care programmes in conjunction with wider MDT
  • Will maintain close liaison with police and secure care staff
  • Will maintain close liaison with all dept staff involved in service users care [both from within the acute trust/HPFT and colleagues in police or secure care] and report on effectiveness and progress to MDT
  • Will lead and / or participate in clinical discussions, case conference etc and will prepare reports on service users as required
  • Will escalate any concerns re patient health or requirements , liase with appropriate medical /psychiatric drs with regards to need for prescribing medication etc
  • Will demonstrate and serve as an example for all clinical activities to appropriate grades of staff
  • To participate in assessment with also formulate management, care planning and review of all individuals, held under 136 in the acute trust ,as appropriate with the relevant disciplines

Person specification

QUALIFICATIONS EDUCATION TRAINING

Essential criteria
  • RMN/RLDN or Dip/ Bsc (Hons) in Mental Health/ Learning Disability Nursing
Desirable criteria
  • ENB 998/ Preceptorship and Mentorship.
  • ENB 812 / BSc (Hons) Specialist Community Nursing Practice (Mental Health) or similar training.

PREVIOUS EXPERIENCE

Essential criteria
  • Post-registration experience with at least some experience at band 6
  • Experience in acute and community areas
  • Experience in care planning, change management and use of Microsoft Office.
  • Evidence of continuing professional development.
  • Knowledge of physical health contributions to Mental Health issues, difficulties, and assessment.
Desirable criteria
  • Experience in working with acute mental health problems in any setting.

SKILLS/KNOWLEDGE/ABILITY

Essential criteria
  • An understanding of the needs of people with acute mental health problems
  • Confidence to work autonomously in the community and use own initiative and make decisions in complex situations.
  • Supervision skills.
  • Experience of risk assessment in an acute / community setting
  • Ability to cope with an unpredictable work pattern and frequent interruptions
  • Ability to work flexibly according to client/service needs
  • Receptive to changing environments and an ability to promote positive approaches to implementing changes according to service and client needs
  • Ability to remain calm in difficult situations.
Desirable criteria
  • Specific clinical skills applicable in an acute community setting e.g. brief therapy, cognitive therapy, and anxiety management.
  • Experience in Service Development.
  • Experience in assisting with Operational policy development

COMMUNICATION SKILLS

Essential criteria
  • Ability to communicate effectively within a multi- professional team
  • Ability to provide and receive complex, sensitive or contentious information, where persuasive, motivational, negotiating, training, empathic or reassuring skills are required.
